Gears of War gets it's "M" rating from its violence. In many occasions, the player can use melle combat to fight other aliens. It's not rare to find blood on the floor whilst playing Gears of War. There are certain locations where there's more blood than paint and the game is overall pretty violent. Blood often sprays on the screen. If you pause the game, go to options, then go to game, you can turn off "Mature Content" which turns blood into sparks and turns off MOST OF the language.
