When the truck crashes into the boat, Elizabeth is knocked unconscious inside the truck. After her flashback, she wakes up several feet outside the truck.
When Andy runs from the fog, the sky is light blue. When he runs inside the house and the fog climbs up the door, the sky is black. A shot or two later, the sky is dark blue.
When the old beach-comber finds the dining room table set up on the beach, the table and chairs are alternately well above the water line and several inches deep in still water between shots.
In a flashback, when the founding fathers put their hands together, three of them stand side by side. When the fourth puts his hand in, a shot from below shows them at 90 degree angle to each other.
When Stevie goes into the lighthouse to work, the bulletin board in the background shows her son's drawings and pictures. They're in different places in the next shot. When they catch fire, they're in different places again.

Factual errors

Stevie receives a package containing a webcam. She connects it to her laptop and is instantly connected to Dan the Weatherman who sent it to her, and they start video-chatting. It takes a lot more than just plugging a camera into a laptop to be video-chatting with someone. Even if she already had the necessary video-chat drivers and client software on her laptop, she'd still need to have Dan's relevant details to connect to him.
When Elizabeth visits Father Malone at the church, the phrase "mene mene tekel upharsin" is painted on the wall but the final word is misspelled "eupharin."

Incorrectly regarded as goofs

Elizabeth notes on the photographs that "something happened" in 1871, and the town sprung up from a few shacks to a thriving community. Elizabeth's photos predate 1871. Many Western US towns started as collections of shacks around springs, mines, or railroads. The "founding" could be the day a collection of shacks became a formal town.
Why is Elizabeth pounding on the church door calling for Father Malone, when the door is padlocked on the outside. Obviously no one could be inside. However, most churches have more than one door so that's not necessarily true.

Revealing mistakes

When the bodies are reeled in from the sea grass, the bottom body blinks for a second.

Miscellaneous

When Elizabeth wakes up in the night at Nick's house, she is making a pencil rubbing of the "scales of justice" hallmark she has been researching. But there is nothing for her to be taking the rubbing from.
Elizabeth uses a lighter on the lid of the old watch , and right away she wipes away the soot with her thumb , the metal would not only still be hot but would have burned her thumb pretty good .
In the end credits, the actor playing the role of Mr Machen, the beachcomber, has been listed as "RNELSONBROWN", without spaces.

Audio/visual unsynchronised

At the start of the boat party scene, Spooner is holding the camera. He say "Hey, yo Sean." to get Sean's attention, followed by "Get your ass up here and take the helm." However, the line "Hey, yo Sean." is clearly dubbed in place of a different phrase.
When Sean rises up off the stretcher in the hospital morgue and accosts Elizabeth, he says "Blood for blood" to her. But his mouth is saying something else (it actually looks like he's saying "All must die" or something similar).

Crew or equipment visible

Tom Malone's stunt double is clearly visible when he's being attacked on the church. The stunt man wears a wig that proves to be unsuited for the character who is slightly bald while the wig has longer hair when he moves.

Errors in geography

There are no large islands off the Oregon coast.

Plot holes

Spooner suddenly reappears in the church where a few people are reunited to escape the fog, and among the people there's Tom Malone, who a few scenes earlier had asked for the arrest of Spooner after the deaths on the boat. Yet when Spooner enters the place, Tom isn't worried about his presence neither says anything to the guy.
Nick hands Elizabeth a camera he found on the Seagrass. She says "It's evidence." Nick responds "I know, it might prove that Spooner is innocent." For that reason, Nick should have given the camera to the police instead of hiding it from them.

Character error

When the lighthouse notice board catches fire, Stevie puts it out with a CO2 fire extinguisher. She holds the nozzle, which isn't recommended with CO2 extinguishers; her hand should've frozen to the nozzle.
When Stevie is talking to Dan just before the fire at the weather station she says "Over and out". It is unlikely that an experienced radio operator would ever say this as the two terms should not be used together. "Over" means the speaker requires a response while "Out" means end of communication.
