Ecks lets the BMW bike fall when he stops in front of the car. It's standing upright in the next shot.
A bus hits, and obviously crunches, a small, metallic-blue car. In the next shot, the car is not damaged.
When Ecks picks the handcuffs, in the police bus, he holds a paperclip with his right hand and picks the cuff on his left wrist. The cuff around his right wrist comes open, but the paper clip is sticking out of the left cuff's lock.
During the end battle at the train depot, Agent Sever's gun changes between cuts.
After Sever loads the "softkill" into the pistol, she cocks the gun and places it on the table. It's clearly not cocked in the next shot.

Factual errors

Robert Gant is portrayed as a civilian Director of the Defense Intelligence Agency or D.I.A. In the real life Director of the Defense Intelligence Agency is a high ranking three-star military officer, Lieutenant General from Air Force or Army or Marine or a Navy Vice Admiral. There's never been a civilian held the position Director of the Defense Intelligence Agency, unless if it's only acting director for a short moment.
Several characters in the film under-react when they're shot with bullets, which is normally a very painful injury.

Incorrectly regarded as goofs

The film states that the FBI doesn't operate in Vancouver, Canada. The FBI often operates in other countries, with the permission and co-operation of foreign governments and law-enforcement organizations. At the beginning, Julio Martin tells Ecks that they will be operating as part of a trans-national task force.

Revealing mistakes

Gant at one point tells Ross that he can tell he's sweating, even though a closeup of the actor's face shows that it's not wet at all with sweat or any form of perspiration.
During the motorcycle chase, the characters pass the same silver Ford Escort, silver Ford Thunderbird, and gold Dodge Aries or Plymouth Reliant wagon repeatedly at different times.
Rifle grenade launchers that attach to assault rifles are almost always single-shot weapons that must be manually reloaded with individual loose grenades after each shot. At several points in the film, Sever repeatedly fires rifle grenades without ever being seen reloading the launcher or carrying any spare grenades.
The pill bottle Ecks uses is quite obviously just a film canister.

Audio/visual unsynchronised

In the covered garage, before Ecks encounters Sever, he manually loads a shotgun round into the already open breech, then moves the slide into place, racking the new round. It is obviously ready to shoot. When he encounter Sever, the gun makes the sound of a shotgun slide being racked, as though there was no round in the chamber.

Plot holes

The bar in the opening scene with Ecks lacks any extras for some unexplained reason.
Ecks pops some pills at one point in the film, saying it's his medication. The pills are never mentioned again in the film.

Character error

Twice during the film, Ecks is knocked flat and props himself up by pressing the barrel of his gun into the ground. That's very poor weapons handling, and any debris in the barrel could cause the gun to malfunction.
