On Halloween 1988, 10 years after Michael Myers tried unsuccessfully to murder his younger sister Laurie Strode (Jamie Lee Curtis) and fell into a coma, he awakens, escapes, and returns to Haddonfield intent on killing his seven-year-old niece, Laurie's daughter Jamie Lloyd (Danielle Harris), who has been adopted by the Carruthers family. When Jamie and her adopted sister Rachel (Ellie Cornell) go trick-or-treating, Michael (George P. Wilbur) stalks them. Wherever Michael goes, however, his psychiatrist Doctor Sam Loomis (Donald Pleasence) is not far behind.
Halloween 4 is the third sequel in Halloween's Michael Myers storyline, following Halloween (1978) (1978) and Halloween II (1981) (1981) and followed by Halloween 5: The Revenge of Michael Myers (1989) (1989), Halloween: The Curse of Michael Myers (1995) (1995), (1998), and Halloween: Resurrection (2002) (2002). (Note that Halloween III: Season of the Witch (1982) (1982) is not a part of the series.) Although it continues with characters created by John Carpenter and Debra Hill for the first two movies, the screenplay was written by Danny Lipsius, Larry Rattner, Bejamin Ruffner, and Alan B. McElroy.
When the screenplay for Halloween 2 was written, they weren't meant to survive. The Michael Myers saga was meant to end with the deaths of Loomis and Michael in that movie, but popularity at the box office brought back the story for another round with Halloween 4 and Halloween 5. In order to bridge the gap, the Ridgemont Federal Sanitarium security guard at the beginning of Halloween 4 tells the ambulance crew, when they come to pick up Michael and transport him to Smith Grove, how Michael and Loomis "nearly burned to death" after their last encounter 10 years earlier. It's already been established in the first two movies that Michael has a supernatural ability to survive, as when Loomis shot him six times at the end of Halloween, yet he survived (and was unimpeded by wounds). As for Loomis, viewers have suggested that he survived the fire either by being knocked to the other side of the room by the initial explosion or by being on the floor below the flaming gas. He is visibly scarred and limping in this movie, supposedly due to the trauma he suffered in Halloween 2.
The crew in the ambulance mention that his only living relative is a niece in Haddonfield. That's when Michael comes out of his coma, kills the crew, crashes the ambulance, and goes on the run.
As Rachel talks with the police, Jamie wanders over to Michael's lifeless body. She takes his hand in hers but drops it when Rachel and Sheriff Meeker (Beau Starr) yell to her not to touch him. As Jamie begins to walk away from his body, Michael suddenly clutches his knife and begins to stand up. The police open fire on him, their shots sending him reeling backwards until he falls into an old mine shaft which collapses on top of him. Meeker takes Jamie and Rachel home where they are joined by Dr Loomis, who assures them that it's over and that Michael is in hell where he belongs. While Rachel rests on the couch, Darlene (Karen Alston) takes Jamie upstairs and begins to run bath water. She lays out some towels and bends over to test the water as someone looks at her through the eyes of a mask. The masked eyes move down the corridor, causing Darlene to look up and scream. Loomis hears the screams and rushes up the stairs to find Jamie standing at the top in her mask and clown suit, a pair of bloody scissors in her right hand. Loomis screams, "No! No!", several times and raises his gun, but Meeker stops him from shooting. In the final scene, Loomis, Meeker, Rachel and her dad (Jeff Olson) stare in horror as Jamie raises the scissors.
