Depending on the episode, the police called Blanche to inform her that her husband was killed in a car accident, or her husband died from a coma brought on by injuries from a car accident.
When the girls refer to the house, the address is quoted as either 5161 or 6151 Richmond Street.
As with any long-running show there are many inconsistencies between episodes. Characters' life histories, their birthdays and ages are reported differently from time to time as the writers invent new stuff for them that wasn't thought of when the series began. "Facts" are altered to fit the jokes, and a good deal of leeway is allowed.

Plot holes

Rose is established as a 55 year old widow whose husband died in 1970 after 32 years of marriage. This makes no sense unless Rose was a child bride. Actually this makes perfect sense. 55 minus 32 is 23, so Rose would have been an adult when she got married at the age of 23. She would not have been a child when she got married. Actually, if she was 55 in 1985 and her husband died in 1970, he's been dead 15 years. And if they where married 32 years when he died, it means she would have married him at 8 years old.
