His stone removed, William is visited by childhood sweetheart Mina, who has marriage in mind whilst his progressive therapy is questioned and the asylum in danger of closing. His colleagues suggest a solution - paying the local innkeeper to feign madness so that William may be seen to 'cure' him - but the plan goes wrong. Mina is in a position to save William's family from ruin but he finds himself falling for Caroline. Meanwhile Robert is summoned to remove the diseased appendix of the Bishop of Lambeth, who mistrusts surgery but John's new lady love Nicola comes to the rescue by drugging the bishop before the operation. William finds out that his family are financially challenged due to bad investments and his father may have to be sent to a public asylum which he would never be able to survive it. A childhood sweetheart Mina comes to the rescue promising to clear the debts but she is angling for William's affections in return.

William's progressive techniques to treat mental health patients comes under scrutiny so he pays the local innkeeper to pretend to be mad. The trouble is the hospital resorts to traditional techniques of cold baths and the spinning chair.

Robert is still dismissive of his wife's ambition and is dismayed that she has been reading the saucy book, The Lustful Turk. Robert does show some surgical skills when he removes the appendix of the Bishop of Lambeth who is in a brothel. John assists with his new girlfriend Nicola who is an apothecary. However the path of true love is blown off course as she is due to marry a wealthy aristocrat.

A busy episode, a lot of things happen including John and Nicola copulating in various ways, taking medication, watching bare knuckled fighting but you just get the feeling that this is as good it is going to get.
