At approximately 8 min, when Vaughn is destructing the car in the driveway, the position of the car changes significantly after he pulls the hood off and throws it into the street. The car is suddenly much closer to the house.

At ~8 minutes when Vaughn tears the hood from the car, he tosses it across the lawn & it ends up in the street but 2 minutes later, a shot from inside the house through a window shows the hood now on the lawn, not in the street.

At ~19 minutes a common continuity error occurs when Vaughn picks up his pregnant wife, her dress is hanging free exposing her bottom for the right camera angle but when he walks her up the stairs, her lower dress is nicely tucked under his arm completely covering her bottom.

Bradley confronts his wife in the driveway and dumps the contents of his purse on the ground. When she goes in the house, there is a long shot of Bradley next to the car, but nothing is on the ground at his feet, where only seconds before there had been the things in her purse.

Factual errors

When the prison bus is backing away from Redleaf, no audible back-up beeper is heard. A vehicle this size would certainly be equipped with a back-up beeper, as required by American Occupational Safety and Health Administration regulations.

Miscellaneous

As Bradley is damaging the car, it's obvious that it was staged to come apart. As soon as he's done damaging it, the car is in a different spot in the driveway.

Incorrectly regarded as goofs

The final scene is obviously a mannequin. As stated in the trivia, the makeup effects were purposefully made to mimic "grind house" style, which often used low budget effects like this.

Revealing mistakes

During the brawl scene, when Bradley (Vince Vaughn) returns to his cell, the body of the slain guard is nowhere to be seen, however Bradley indicates verbally to Warden Thuggs (Don Johnson) that the guard in question "will not answer", implying that the man has not been moved and that he is, in fact, dead.

When Bradley (Vince Vaughn) is lying on the floor after the first assault on correctional officers, his head changes positions from the time the big guard is directed to "administer justice" until the guard stomps on Bradley's head - his head changes from lying on his shoulder to lying directly on the concrete floor.
