There are a few scenes with dead bodies at a hospital, one in particular shows a corpse with the chest cavity missing, this is shown a couple times but the blood shown is quite minimal. A few fight and shootout scenes, but overall the violence is fairly limited.
Blood from an accident is shown. Grazes, bruises, scars and open wounds shown.
A man is pepper sprayed in the eyes.
A man gets a syringe in him to sedate him whilst being aggressive to hospital staff.
A man is shot in the knee cap.
A man chokes another man until he cannot breathe and becomes unconscious.
Carcuses are shown in a lab/morgue where the organs have been removed from the bodies. Quite graphic and gory scenes. One scene shows a dead body with eyes gauged out.
A man grabs a gun from a police officer and takes someone hostage at gun point.
An explosion occurs with four people being caught in it. You see the fire engulf the persons.
Two men have a fist fight where punches to the face, stomach and throat are shown.

A man get a syringe in him by hospital staff to sedate him as he is being aggressive. The man later injects himself three times with adrenaline to come off the sedation he has been given. A man gets pepper sprayed in the eyes. A woman complains to a man that she has been injected with a sedative.

The movie has a very uneasy and dark atmosphere throughout.
A little girl is charged by a vicious looking dog. Her father tries to hit with a rock, but she moves backwards in fear and falls on some construction site, with her father falling with her to rescue her.
The hospital staff insist that they have never seen the main character's wife and child, where they say he probably "imagined" them. Viewers are made to think that the main character is set up in a plot or cabal perpetrated by the hospital staff and the cops as well. This concept is pretty much the storyline of this movie, and viewers might it very intense.
Suggested MPAA score: Rated R for some violent/disturbing content and language.
