A man is approached at an outdoor cafe. It would seem he owes money to some kind of loan shark. The man tries to run, but the chaser hooks up with another guy, jabs him with a hypodermic and the main is dragged into a hospital. A doctor leans over him and says "Sorry Mr. Wyatt, you are out of time." The next morning his body is found in an alley, his heart ripped from his chest. (Ouch!)
Liz has a terrible nightmare that Tom invaded her motel room and told her to ask Red about the night of the fire. Red enters and shoots Tom. He asks Liz, "What do you really want?) Liz wakes up with a start. As she leaves her motel room she notices a man with a backpack behind her. Paranoid she forces him up against a wall and frisks him at gunpoint. He's clear. She apologizes and he goes on his way.
Red meets with Niko and tells him the Indonesian government is planning on turning its day-to-day operations over to a private company and he wants that company to be the Syndicate. Niko is skeptical and says the other members will be too since this could bankrupt them. Red says to take it to the table anyway.
Liz and Dembe show up and she asks where Naomi, Red's ex-wife, is. He changes the subject to the dead, ripped out heart guy Wyatt. Red says this is the work of one Dr. James Covington (#89) who is operating an illegal organ transplant ring for terrible criminals or super wealthy people who can't or don't want to go through dangerous channels.
At the Post Office they run him down noting that he used to be well-respected and then tried a risky transplant of adult organs into a child that failed and he vanished.
Cooper also brings Samar Navabi in and formally introduces her as a new member of the team, on loan from Mossad. Even though she saved their lives in Poland, Ressler and Keen are wary. Aram, however, is clearly smitten. She is smitten by their technology.
Liz and Ressler learn from the man's widow the truth about Covington: The organs are not purchased, they are rented. People get the organs and then have to pay a half million dollars a year to keep it. If they run out of money, they lose the organ.
Niko takes Red's Indonesian offer to the other Syndicate leaders. In the middle of their meeting a squirrelly man -- played by Pee Wee Herman-- named Fargas who claims to work for Berlin and says he has a counter-offer on the Indonesia deal that they would be stupid to refuse.
Liz and Ressler track down a man who is a medical examiner who moonlights as Covington's organ man-- he takes them from bodies and then hands them off to a messenger.
Niko tells Red about Fargas and says they're worried and it is a good proposition. But Red says he's feeling bullish. He tells Niko to keep an eye on the others.
Liz and Ressler get the M.E. to make a decoy organ hand off with a messenger. But the messenger makes Liz and Ressler following him and he takes off on his motorcycle. They give chase and the messenger ends up getting creamed in traffic.
Niko comes to Red and shows him pictures of the other Syndicate leaders meeting with Fargas, presumably taking his offer. Red confronts them and has them taken away and thanks Niko for ratting them out.
Aram tracks down the person who was supposed to get the [fake] donor organ that the messenger was carrying. Of course he's an old frenemy of Red's who tracks him down and strongarms by way of poison into telling them where Covington is.
Cooper talks to Liz about her psych evaluation, saying the doctor was concerned about her and that she is "fine." Liz says she'll mourn and rest once they nab Berlin. Cooper says he isn't suspending her but he is worried about her crazy, paranoid behavior and wants her to know if she needs to talk he's there.
Niko goes to Fargas, he was double-crossing Red! Niko says because of that he wants Berlin to kill Red to protect him for this betrayal. Fargas tells him to set a meeting with Red and he'll take care of it.
They track down Covington's secret surgery warehouse and are surprised to see families with children in the waiting area. They go to arrest Covington but he is in the middle of a surgery on a young boy and if they take him the boy will die. Ressler seems not to care but Liz says she knows they wouldn't be able to live with that. It turns out that Covington was using the money he was receiving from the wealthy and the criminal.to rent organs to pay for organ donations to children who needed them. So for him the ends justified the means. not so much for the law and he is arrested. The parents of the boy thank Liz and Ressler for waiting.
The meeting with Red goes down and it turns out Red was actually double-crossing Niko! And the other Syndicate guys were in on it! Turns out Fargas is just an old buddy of Red doing an acting job. he takes the gun from Fargas and shoots Niko-- after a long monologue about how he got everything to work out in Indonesia. Cold-blooded that Red.
Later at the Post Office Samar tries to bond with Liz and asks her out for a drink to celebrate nabbing Covington. Samar knows she's wary but says they're working together now and she's right not to trust her. Liz tells Samar she is wrong that she found Red, Red let her find him and for that he had a reason and that's what she really wants to know.
She heads back to the motel and apologizes to the guy. He's British, and very understanding. And then he heads back to his motel room and, of course, it turns out he's a bad guy. He opens a bag revealing a sniper rifle and checks out the scope.
Red goes and meets with Naomi at her safe house. He tells her she looks different. She says not as different as him and promptly punches him in the mouth.
