(at around 8 mins) When Madison's husband hit her in the head, the wall has blood on it. After she stands up, there is no blood on the wall.
(at around 1h 18 mins) When Sydney first sees the Simion Research Hospital it is daylight. Despite the short distance, when she arrives there it is the middle of the night (notice the full moon).

Factual errors

(at around 34 mins) Early in the film, Madison is shown to be listening to a radio station with a frequency of 90.8. Since the film takes place inside the US this is impossible because the FCC makes all operating radio stations end their frequency in an odd number to guarantee that all stations have a 200 kilohertz slice and that they can thus avoid interfering with each other.

Incorrectly regarded as goofs

(at around 58 mins) Detective Kekoa Shaw finds 5 pen drives dated 1992 when pen drives were only invented 5 years later in 1998 by IBM.

However, it is entirely possible that the drives were purchased later, and merely have files on them that are from 1992 (perhaps after an older storage medium was disposed of, or just when the Dr. decided to add backups of her older files).
Despite Hollywood's insistence on always portraying Seattle as having thunderstorms and a torrential downpour, they are both actually extremely rare events for the city and also very unlikely to be immediately followed by a bright and sunny day.

Rare and unlikely do not mean impossible. Not a goof.

Revealing mistakes

(at around 1h 28 mins) Right before Madison is attacked by her cell mate, she reaches for the back of her head before her attacker grabs her by the hair. Since her back is turned, there's no way she could've anticipated that, and so it's pretty clear the actress wanted to make sure her wig didn't fall off.

Plot holes

When Det. Shaw is reading medical journals, as he flips the pages he comes across 5 flash drives in the middle. He would have heard them clack around when he picked up the journal since there are packed loose in a cut out area within the journal.
