The scene where Maggie must perform a C-section on a still-living Lori in order to save her baby was so troublesome for actress Lauren Cohan that she nearly quit the show. It wasn't until a talk with co-star Steven Yeun that she changed her mind and continued with the show.
For the C-section scene, Sarah Wayne Callies (Lori) drew on her real life childbirth experience, which she described as "a battle." Sarah says Lori always saw her pregnancy as a death sentence.
They used a radio-controlled prop baby with moving arms and legs for the birth scene.
The memory of the fallen in this episode is honored by replacement of regular music with utter silence during the entirety of the end credits.
Six-week-old twin sisters (Adelaide Cornwell & Eliza Cornwell) were cast as the baby, Judith Grimes.
The fifth highest rated episode of the series as of February 2016, behind The Walking Dead: No Way Out (2016) (Season 6x09), The Walking Dead: Too Far Gone (2013) (Season 4x08), The Walking Dead: No Sanctuary (2014) (Season 5x01) and The Walking Dead: Thank You (2015) (Season 6x03).
Lauren Cohan (Maggie) was holding a real six-week-old baby girl (played by twins Adelaide Cornwell & Eliza Cornwell) in the scene where Maggie and Carl meet back up with the group with a newborn baby.
Carol (Melissa McBride) was originally going to be killed off in this episode, but the producers decided to scrap the idea.
Michonne (Danai Gurira) suggests to Andrea (Laurie Holden) that they head for the coast and hopefully find a boat. Like in the TV series, Fear the Walking Dead (2015), Victor Strand is the only other character in the two Zombie Apocalypse shows to suggest this to their group.

The first set of twins to play Judith Grimes (Adelaide Cornwell & Eliza Cornwell)'s uncle, Brendon Cornwell, also had a role in The Walking Dead (2010) - later playing a Woodbury survivor in episodes 3.9 (The Walking Dead: The Suicide King (2013)), 3.10 (The Walking Dead: Home (2013)) and 3.16 The Walking Dead: Welcome to the Tombs (2013).
