The nightime establishing shots of the fictional "St. Brendan's Medical Center", are actually shots of Hahnemann University Hospital in Philadelphia, Pennsylvania, even though the show is set in New York City. This is more obvious in some shots, which actually include Philadelphia City Hall in the background, topped with the statue of William Penn.

Perlman and Hedaya also played ex's Carla and Nic Tortelli on Cheers (1982), who are the parents of four kids, where Perlman's character Carla again had nothing good to say about Hedaya's character Nic, who also left her when their kids were young.

In season three, episode sixteen, "Lahiri Family Values", Mindy (Mindy Kaling) and Rob Gurglar (Tate Ellington) go to the bank in San Francisco to apply for a small business loan. The street shots around the bank were filmed in Toronto, Ontario, not San Francisco. Specifically, they were filmed at the corner of Yonge and Queen streets, which are easily identified by the distinctive street signs.
