In the original script which was written in early 2011, Selene's Death Dealer outfit is not in the same room where she is after she wakes up, and instead she wears a thin medical gown, similar to the one that main character Alice wears in first few scenes of Resident Evil: Apocalypse (2004), throughout most of the story until she finds similar outfit in vampire coven and changes into it after the gown starts falling apart from filth and blood following the previous action sequences.

First version of the script was titled Underworld - New Dawn, and before it went through at least half a dozen screenwriters, some of who were left uncredited, it was very different from the film. Some of the differences include;

Antigen was originally called BioCom; Selene wore her iconic outfit from previous two films only in opening scene before she's captured but near the ending she finds and wears similar outfit; She was imprisoned by BioCom for 15 years instead of 12; Her daughter was 14 years old instead of 11 and she was called Nissa: It wasn't Nissa's DNA BioCom were after, instead they wanted to use them both as breeders for super hybrids; Selene escapes during attempt by BioCom to impregnate her and not after waking up from cryogenic suspension; Super-lycan wasn't as big and instead he was lot more like William from second film; There were no tunnel lycans and Sebastian's character. All the later versions kept changing so much of the story that even when filming started there was no finalized plot storyline.
