Continuity
When Jay Baruchel and Craig Robinson are running from the dog-like demon in the neighbor's house, Craig saves Jay by pushing him out of a glass door. As Craig is jumping into Jay he is securely wearing the backpack full of food but when they crash through to the outside the very next second he is not wearing it.
When Jonah Hill vomits on Seth Rogen and James Franco, they are completely covered in vomit. In the next shot their hair is dry and their t-shirts are only a bit wet.
When Emma Watson is trying to escape the group after overhearing the rape conversation, she hits Seth Rogen in the face with the butt of her ax, breaking his glasses in half. While he wears them with tape around the right temple and hinge for the rest of the film, the glasses clearly broke in half at the bridge, which would require tape on the bridge to hold them together.
Approximately 56:23 into the movie, the camera is showing James Franco on the floor above and the backs of Danny McBride and Seth Rogen. Rogen is visibly cracking up. Yet, when the camera is showing their faces, they're straight-face.
Incorrectly regarded as goofs
James Franco knows that the gun they have is a prop, but he panics with everyone else when Jonah Hill pretends he is going to kill himself with it. However, it is clear that he wants to keep up the illusion for everyone else that it is real.

Audio/visual unsynchronised
When Jay Baruchel and Seth Rogen make it to Heaven and see Craig Robinson, Jay starts yelling, "Yay!" as both Jay and Seth run to Craig. Jay yells the whole time until they reach Craig, but when they reach him the audio for Jay is still going while his mouth is closed.
Character error
When the Backstreet Boys start their dance choreography in Heaven, the band member second from the left can be seen messing up the choreography, raising the incorrect arm motion at the start and having a concerned look on his face.
Continuity
When they first break through the floor/ceiling, there is no rebar in the concrete (which they would not have been able to break with their improvised tools anyway). When the hole is next shown a few sequences later, some severed stumps of rebar have magically appeared.
When Jay Baruchel and Seth Rogen arrive in Heaven, Craig Robinson gives them halos above their heads. In the next scene as these three characters join in to dance with the Backstreet Boys, their halos are no longer above their heads.
Revealing mistakes
All concrete ceilings are always reinforced with steel nets ('rebar') specifically so that NO big chunks can just break away and people suddenly drop through large holes.
After Craig Robinson gets taken into Heaven they are seen in James Franco's Prius. All of the light are on in the dashboard indicating that the car just in accessory mode and not actually running. This means that the car couldn't move because you couldn't put it into drive.
