Deadly cosmic rays bombard the U.S. Army base where Bruce Banner is working with the powerful new element Perium 99. The rays cause Banner to change into the Hulk at random, and cripple the base's communication and defense systems, paving the way for Doctor Octopus to invade the base and steal the Perium 99. It's up to the Hulk to stop Doc Ock from mutating every person on earth into hideous creatures, and blackmailing the entire world.
