- It's Jack's first birthday, so Jordan's siblings Dani and Ben are in town. Dr. Cox tells J.D. to leave a patient for half an hour and check up on Ben's leukemia, but when a patient dies, Dr. Cox blames J.D., and takes over his patients. Carla and Turk argue about Turk getting rid of his mole and Carla taking Turk's last name. The main theme is acceptance.—KTW / StevoRamone
- Jordan's brother and Dr. Cox's BFF Ben (played by Brendan Fraser), returns from his, as Doctor Cox puts it, his "world leukemia tour". We learn that he returned so that he could come to his nephew's (Jordan and Perry's son) 1st birthday party. While we wait for the upcoming event, we find that Dr. Cox has been working 60 hours straight. The reason, he states is "people die when i'm not here". As Perry continues working, Ben continuously follows him around, joking and playing pranks on fellow hospital staff. Ben also helps Perry learn to not blame other doctors (JD, in particular) when patients die. As the camera goes from one castmember to the other to show how each one has learned to accept things they cannot change, you will notice that each one of them is getting dressed in somber, formal clothing. The next scene shows Dr. Cox, also in formal-wear, walking along with Ben in what looks like a park. Dr. Cox jokingly asks Ben if he should have dressed better for the occassion. After Ben tells him that he's dressed fine, Dr Cox ask him if he brought his camera with him to take lots of pictures of kids covered in cake and chocolate syrup and balloons and stuff like that. The camera pulls back and we see JD in the background. JD, looking solemn, asks Cox where he thinks he is right now. The camera cuts back to Cox, who looks around and realizes Ben is not there. Cox (and the viewers) finds that they are not at a birthday party in the park, but in a cemetery. The next scene shows JD and Cox in the front row of a funeral, where they are soon joined by the entire cast. The camera then shows the flower-laden casket which has perched next to it, a picture of the deceased, Ben Sullivan. The camera cuts back to Cox as Jordan and JD put their arms on his shoulders as he quietly weeps at the loss of his best friend.
