Linda Bloodworth-Thomason came up with the idea for the episode's plot and title while caring for her mother Claudia, who had contracted AIDS from a tainted blood transfusion and died of the disease shortly before Linda wrote the episode, the title of the episode came from a comment "The good thing about AIDS is that it's killing all the right people.", which was paraphrased in the show's script.
