Each time George mentions Millie's older brother Luke, the movements of her mouth don't match what's being said. It appears that the name was changed and then dubbed after-wards.

Continuity 

When George asks Betty where she got her ring, Betty replies that she saw it in a store in 1927 and that her boyfriend bought it for her. When Betty looted her own grave to get the ring back, her tombstone read '1899-1926' Meaning that should would have already been dead when the ring was purchased and it wouldn't have been on her corpse.

When Mason is with the old lady, he goes into the kitchen and picks up some sugar cubes and puts them back in the bowl. In this shot the old lady's body has her hands by her head. When we see the old lady's body from the dining room, where her soul is, her arms are stretched out much further past her head than they were in the close up shots.

When Joy is sitting on the toilet talking to Reggie while in the bathtub. The shower curtain is shown four times as a brown butterfly pattern and then the fifth time it is shown as a brown cat pattern which makes more sense, since the bathroom has a cat theme.

Errors in geography 

George's "reap" is an Irish man named Patrick Cassidy, who is celebrating his birthday at an Irish pub, and dies while dancing an Irish jig. After he has died, his soul sees ocean cliffs appear as his gateway to the afterlife. The character says they are the Cliffs of Dover (which are on the southern coast of England), but the cliffs shown look nothing like the Cliffs of Dover. They look much more like the Cliffs of Moher, which are on the west coast of Ireland, and would be a more logical place for an Irish character to remember.

Plot holes 

When George gets into the lift at her office, her reflection is very obvious on the sliding doors, but we were told back in the pilot that Reapers have a different visage to the outside world that shows up in reflections, on camera etc.
