A mysterious illness that robs the 4400 of their abilities while also making them deathly ill begins to manifest. As the illness begins to spread, the government orders NTAC, temporarily being headed up by Dennis Ryland once again, to round up all 4400 and place them back in quarantine. As the situation quickly escalates to a possible confrontation, Tom and Diana race to find a cure.
