Character error
(at around 9 mins) The message written on the ceiling ("Look closer, Detective Mathews") misspells Eric Matthews' surname.
Continuity
(at around 51 mins) When Xavier is trying to open the door that was timed four minutes, there is no number on the back of his neck.
(at around 18 mins) When Jigsaw is explaining Daniel's situation to Eric, the clock on the wall says 7:30. After the timer in the monitor room has gone down by 90 minutes (at around 59 mins), a shot of the wall clock shows the time as 8:00 - only half an hour later.
(at around 1h 11 mins) The first time we see the number 9 on Addison's neck, it has a curvy line, looking like an upside-down 6. Later, when there are flashes of the previous numbers (at around 1h 21 mins), the 9 shows up with a straight line.
(at around 14 mins) When the SWAT team have their guns aimed at Jigsaw's heart (just after Matthews says "Hey - is this close enough?", we can see three laser guides on his chest. Then one of the SWAT team members points his gun down, but there are still three laser guides on Jigsaw's heart. Seconds later there are two lasers, then three again.
(at around 35 mins) When they discover Obi's envelope, the knife's position on the envelope shifts between shots.
(at around 35 mins) Laura enters the room with a clean nose; in the next shot, she has blood running from her right nostril. In the following shot, it has disappeared.
(at around 19 mins) Gus' tie changes from being sloppily pulled to the right in select shots as they wake up and then being straight in other shots.
Rigg's earpiece disappears between shots.
(at around 16 mins) When Eric meets John, his badge is normal. A few moments into the scene, it becomes twisted and when it cuts to John then back to Eric after a second, the badge is normal.

(at around 12 mins) The crew is reflected in one of the large white SWAT vans that pulls up outside the steel factory.
Factual errors
(at around 12 mins) A SWAT member uses a battering ram to open the outside door of the facility, but the door opens outwards.
(at around 24 mins) To fire a revolver, the hammer must fall forward to drive the firing pin into the cartridge's primer. When the key is turned in the door, we see the booby trap mechanism begin to cock the hammer of the revolver. But just as the hammer reaches its full rearward position, and before it falls forward, the revolver fires. Then the hammer falls forward.

Audio/visual unsynchronised
(at around 56 mins) In the Syringe Pit, when Addison says, "Stop this bullshit!", her lips are saying, "Fuck this bullshit!"
Continuity
(at around 1h 17 mins) When the body of Zep from Saw is shown in the bathroom, there is no evidence of his head being smashed in by the toilet tank lid.
The wounds on Amanda that were inflicted in the Syringe Pit disappear and reappear throughout the remainder of the film.
(at around 2 mins) When Michael is watching the tape of the puppet explaining his situation, the channel indicator on the television keeps changing from displaying channel 3 in one shot to being covered up with duct tape in the next.
(at around 50 mins) When Xavier opens the door to the room containing the "Needle Pit", it shows a very short clip showing the timer on the door for the antidote. If you slow the clip down, you will see that the timer shows "4:00"; in the next scene, it shows "2:58".
(at around 1h 19 mins) In Saw, Adam was chained by his left ankle, but when his corpse is found in this film, it's his right ankle that is chained.
(at around 4 mins) In the extreme close-up shot of Michael preparing to gouge out his eye, the blood covering it disappears.
(at around 24 mins) When Gus is shot through the eye, the bullet goes through his left eye, leaving blood running down his face, yet later on in the film, when Xavier sees the number on the back of Gus' neck (at around 1h 2 mins), we see Gus' eye is perfectly normal-looking, with no blood or bullet wound.
(at around 27 mins) The pool of blood from Gus' gunshot wound changes when Daniel goes to cover him with the coat.
Incorrectly regarded as goofs
(at around 1h 9 mins) When Xavier enters the room where Addison has her arms stuck in the "Razor Box", blood is suddenly all over her hands; when she put them in there was no blood. There is no way blood can cover her hands like that just by getting stuck in the box.
Plot holes
(at around 5 mins) In the opening scene, Michael drops to his knees and screams "NO...NO...NO...NO...!" right before the "Death Mask" closes on his head. In Jigsaw's video, he never told Michael how much time he had to save himself, and the timer was on the side of the device where Michael couldn't see it, yet he somehow knew exactly when his time was up.
