When the grasping surgeon demands an extra $300 [perhaps £2,400+ in today's terms] for 'traveling expenses', or he will refuse to operate on Francis' wife, one reflects that if this threat actually goes through he will not only lose the 'expenses' (which should have been included as he has presumably paid for his transport) but also his fee for the operation.
