Incorrectly regarded as goofs
The burn victim woman Cole talks to at the end (when Stanley walks in on him in the dressing room) was presumably a teacher at the school. In the very next scene Stanley says, "You know, when I was a student here there was a big fire." Obviously the burn victim woman (or a different fire victim we don't see) told Cole about Stanley's stuttering problem.
Cole says, "They only see what they want to see," which neatly and intentionally explains why Bruce doesn't see his own blood until he realizes he's dead. Likewise, ghosts constantly interact with the real world, hence Cole's scratches, the poisoned girl's ability to give Cole the VHS tape, etc. Regarding Bruce's clothes, it appears he only adds/removes his jacket, but either way the explanation that he sees what he wants to see should be sufficient.
People are clearly shown walking around prior to the shot in the car. They are walking toward the accident that Cole is talking about. No one is shown in the surrounding cars because they have already exited the cars to look at the accident.
This is not at all the progression of the action in the film. When Malcolm comes in from the street, his wife is already asleep. He talks to her and she drops the ring. At this point, having paid attention to what Cole said about dead people "only seeing what they want to see", the viewer realizes that Malcolm sees the furniture the way it used to be. Then he realizes that he's dead and begins to see things the way they are: at Cole's, the restaurant, the blocked door, the blood and finally, the re-arranged furniture.
