Goofs

Continuity 

Many character's names were changed in the dub, yet when porting the series to America, many references to their Japanese names were not removed/edited out/etc. For instance, Unazuki Furuhata is called "Elizabeth" in American versions of the show, yet her doorbell, even in the dub, clearly says "U. FURUHATA." Likewise, Michiru's name is changed to Michelle in the dub, yet at the end of one episode, her journal clearly has "Michiru" written on the cover.

Michelle says "We're cousins. We grew up together." and Amara says "We've been inseparable since birth". Later it says they were cousins who grew apart.

In the English dub, Amy's last name is Anderson in the early seasons. But in the later sagas, it is changed to Mizuno (the latter is accurate to the original Japanese version.)

Incorrectly regarded as goofs 

When Rini fell on the ground, a boy said she wanted to show her something. When he took off his shorts, he said "I wear Sailor Moon undies!" In the next shot, his underwear is plain white. He was most likely just making a sarcastic joke (the dialogue was changed for the edited dub, because in the original Japanese version, he is not wearing underwear at all, and refers to his penis as his "little elephant.")
