Before he's supposed to testify against the Mafia and get into a witness protection program, mobster Joey Arnello collapses. It's not sure whether he's faking it or if he's really in a coma and House gets court ordered to find out. While he and his team do their best to find out what's going on with Joey, and while Joey's brother Bill tries to keep Joey in the hospital so he won't testify, Cuddy tries to convince Vogler House is an essential part of the hospital. The battle with Vogler (Chi McBride) continues as Cuddy desperately tries to save House and Herself. The cost increases as she wins a reprieve.

House is trying to save a mobster (Joseph Lyle Taylor - Girl 6, Summer of Sam) from the government while his brother (Danny Nucci - Titanic) continually threatens the doctors. The discovery of the reason for the brother's illness is extremely difficult for his brother to except. The fact that he has Hepatitis C is only a small part of the problem, and he has to decide what is most important.

Cameron (Jennifer Morrison) had to take Foreman's digs as it is discovered that she really likes House.
