When Alex's brother Luke is admitted to Hope Zion Hospital in critical condition, Joel and Charlie must work together to keep him alive while keeping his admittance a secret from Alex. Meanwhile, Alex figures out a way to remove a tumour from a patient pregnant with twins, while Maggie is thrown off her game by the patient's gorgeous OB/GYN.—MissFortuneCookie

Alex (Erica Durance) and Maggie (Julia Taylor Ross), with the help of gorgeous Obstetrician/Gynecologist Dr. Kalfas, must remove a tumour from a patient who is pregnant with twins, but encounter complications when they discover that the tumour is uncomfortably close to the uterus. Throughout the day Maggie is thrown off her game by Dr. Kalfass charm. She seeks Gavins (Kristopher Turner) professional help in dealing with him, but as the day goes on, the two of them discover theres more to the OB/GYN than meets the eye. As Alex performs her surgery, Joel (Daniel Gillies) receives news that her brother Luke (guest star Tyler Hynes, WAREHOUSE 13) is in critical condition after a 40-foot fall. When Luke arrives at Hope Zion, Joel works with Charlie (Michael Shanks) to help Luke, who has a metal pole protruding from his leg, all the while trying to keep his admittance a secret from Alex. As unanticipated, complications arise and Charlie is forced to deal with Lukes spirit, whos less than easy to talk to and whose presence brings all kinds of emotions to the surface, eventually causing Charlie to admit his biggest secret to Alex.
