The film contains a realistic and intense depiction of horror that is concentrated towards the end of the film.

The impactful scenes include the portrayal of a victim's head being pulled towards the ceiling with his neck stretched unnaturally, as well as a woman's decapitated legs.

The battle scenes between the two spirits, Kayako and Sadako, in the haunted house and subsequently in the forest are prolonged and intense, but can be suited for an older teen audience.
