Continuity 

When Rachel sees the article about Megan's disappearance/murder her age is listed as 27 however, at the end her grave stone says "1990-2016" making her 25 or 26 depending whether she died before or after her birthday.
The action in the tunnel takes place when it is dark but after that when Tom takes Meghan out to the woods, it is light out.
When Rachel is filming on her cellphone, she does in horizontal way. When she looks at the recording it's like it was done in vertical way.

When Rachel goes to Scott's house and he gives her coffee, her coffee cup changes position on the table even though she hasn't touched it yet.

When Anna is trying to figure out Tom's Windows password, the password is visible. Windows obscures the password so that it's not visible.

Miscellaneous 

When the family takes the selfie in the beginning of the movie, they use the wrong camera, and worse, they use forgot to disable the rear flash.
