- Alice's stories about Wonderland get her admitted to an asylum.
- A hole erupts in the ground and a young blonde girl in a blue and white dress, Alice, climbs out. She runs down a lane, calling excitedly for her father. When she reaches her house she bangs on the locked door. Her father answers, and is shocked to see her. He tells her that she was gone so long that everyone thought she was dead.
Some time later, Alice through a keyhole as her father talks to Dr Lydgate about the strange stories she's told about her time away. Dr Lydgate insists that she's delusional and he can treat her. Alice insists that she's not a liar and she can prove it.
In modern day Storybrooke, the Knave of Hearts walks down the street, almost getting hit by Emma's VW bug. Ashley is closing up Granny's cafe with Leroy. He pushes past them as they leave and then breaks into the cafe and makes himself a cup of coffee. As he stands there drinking a rumbling occurs and a hole explodes in the floor. The White Rabbit pops out of it and tells the Knave he has to come with him. The Knave says he doesn't care about whatever the Rabbit is interested in, but the Rabbit says that it's not a what, but a who: Alice.
In the 19th century, an adult Alice is an inmate at Bethelem Asylum where she lives under the care of Dr Lydgate. He questions Alice before a panel of doctors to determine if she still believes in the fantastic stories she has told, or if she is "cured" and can be released. As he reads some of her fantastic claims she flashes back to events in Wonderland.
On the run from royal guards, Alice steals part of a size changing mushroom and flees into a hedge maze. She eats the mushroom, which causes her to grow very small. As the guards rush obliviously past her she is knocked aside and lands near a bottle. She climbs inside for refuge only to find it inhabited by a Genie named Cyrus. As they talk, Alice says that she has finally found proof to show her father that she was not lying and pulls the White Rabbit out of her bag.
Lydgate reads about how she claims to have had all sorts of adventures with Cyrus and eventually fallen in love with him. Alice flashes back to her and Cyrus standing on a cliff above the Boiling Sea. Cyrus says that he had been a prisoner of his lamp, forced to obey other people's wishes, but that she has freed him and allowed him to wish for what he wants. He asks Alice to marry him and she says yes. Before they can do anything a group of guardsmen show up with the Red Queen and attack the pair. They briefly seem to capture Alice and Cyrus, but Cyrus breaks free and rescues Alice. However, the Red Queen waves her hand and knocks Cyrus off the edge of the cliff. Alice watches horrified as Cyrus plunges into the steam of the Boiling Sea.
In Bethlehem, Alice claims to not believe any of her stories. But Lydgate reveals that she still cries out for Cyrus in her sleep. He advises her that there is a new procedure which could remove all her memories of Wonderland and cure her of the delusions which afflict her. Alice agrees to the procedure and signs the appropriate papers.
Alice lays in her cell, trying to sleep. She is surprised when the door opens and the Knave of Hearts appears. He tells her that she has to come with him, but she refuses, telling him that she wants to forget. Orderlies show up and grab the Knave, trying to drag him from the room. He resists, and tells Alice that the White Rabbit saw Cyrus and he's alive. Alice, suddenly motivated, beats up all the orderlies as the Knave watches. She then grabs her boots and storms out of the cell.
They flee to the bowels of the asylum where they meet the White Rabbit. As they go to leave the asylum, Lydgate shows up and begs Alice to stop, saying he can cure her. He is amazed when the Rabbit steps from the shadows. Alice, the Knave and the Rabbit flee the Asylum and run through the streets of London chased by police. Arriving, in a dead end alley, Alice tells the Rabbit to dig a hole. He opens a portal and jumps through. She prepares to leave, but the Knave balks saying that he is a wanted man back in Wonderland. She tells him that he owes her because she helped him find his heart. Reluctantly he agrees to go with her and the two jump through the portal.
They arrive in Wonderland and fall out of the sky over a marshmallow lake. The Rabbit is on a dock, but Alice and the Knave land on the lake. Carefully making their way across the mallow they talk to the Rabbit, who reveals that he didn't actually see Cyrus, but merely talked to the Dormouse, who had seen him. According to the Dormouse, Cyrus is alive and was staying at the Mad Hatter's old place which has been abandoned. The Knave wants to leave and return to Storybrooke, but Alice insists they stay and search for Cyrus. Suddenly, they notice that they're sinking into the lake. The Rabbit runs off to get them help as they sink further in. Just before the Knave's head sinks into the mallow, Alice uses one of the fire breathing dragon flies to burn the surface of the lake, creating a hard surface which they can use to escape.
After cleaning off, Alice decides to head through the Tulgey Wood to get to the Mad Hatter's place. When the Knave once again balks at accompanying her she offers to pay him. She opens a secret compartment in the heel of her boot and pulls out three red gems: wishes. She offers to give him the wishes if he helps her find Cyrus. When he asks why she doesn't just use the wishes to get Cyrus back she explains that wishes often backfire and have unintended or ironic consequences to them and that she can't risk using them to find Cyrus. The Knave agrees to help her.
Meanwhile, the Rabbit runs down a path and encounters the Queen and some her guardsmen. He tells her that he got there as soon as he could. She takes him back to her palace where she tells him that he has to be her ears and report back to her everything Alice does.
In the Tulgey Wood, Alice decides to climb a tree to get a better view of where they are. She takes off her boots and climbs the tree. The Knave looks at her boots for a long moment and then steals them. Up in the tree, Alice sees the Hatter's house. But when she turns around the Cheshire Cat materializes before her. The gigantic cat says that Wonderland has become stranger since Alice left and that he has become stranger too. He says that there's nothing left to eat in the forest except for her and takes a swipe at her. Alice falls out of the tree and the Cheshire Cat follows her. After a brief fight, the Cat pins her to the ground, saying that the Red Queen told him how sweet Alice would taste. Before he can eat her though the Knave jumps on the Cat's back, distracting him. When the Cat attacks, the Knave throws a piece of size changing mushroom into his mouth, causing him to shrink to the size of a house cat. Alice berates the Knave for trying to steal from her and explains that wishes cannot be stolen, but only granted. He agrees to continue with her on her quest.
At the palace, the Queen goes out to her balcony and speaks with the wizard Jafar. He tells her that he has a long term plan for Alice and Cyrus but that she has played her part by getting Alice there. He then uses his magic to begin to choke her to death. But the Queen reminds him that he doesn't know where Alice is, while she does. She says that he needs her help and Jafar reluctantly leaves on his flying carpet.
Alice and the Knave arrive at the Hatter's house but find it deserted. The Rabbit arrives and tells them that maybe the Dormouse was wrong. The Knave suggests that Alice move on, return to her home and find someone else. She storms outside and finds Cyrus' amulet on the ground. She shows it to the others and the Rabbit notes that if the amulet survived then Cyrus might have too. Reluctantly, the Knave agrees to help Alice and the Rabbit try and find Cyrus.
A flashback reveals what happened to Cyrus. As he fell off the cliff he plunged into a cloud of steam and was caught by Jaffar's flying carpet. In the present day, Cyrus wakes up in a cage, hanging inside Jaffar's fortress.
