Continuity 

The year of birth of Oliver's father, Robert Queen, is indicated as 1948 on the TV news item confirming his death, whereas the tombstone at the Queen mansion shown in this and subsequent episodes reads 1958.
While Oliver is standing on a table at his nightclub party, he lifts a shot of tequila to his lips with his right hand. But when the angle shifts, he brings the glass down in his left hand. The angle shifts again, and the glass is back in his right hand.

Plot holes 

An arrow is shown, revealing how Oliver took the money from Hunt. It is clearly visible, and it is incredibly unlikely that it would not have been noticed.

Revealing mistakes 

When Oliver and Tommy pull up outside the old Queen Industries factory, the logo's reflection in the car window reads left to right instead of being reversed.

Character error 

When Robert Queen commits suicide in the life raft, to allow his son to survive with the supplies, he fires a shot through his skull at a downward angle. This would certainly result in the bullet penetrating the skull and exiting with sufficient remaining velocity to pierce the hull of the craft, sinking it and killing Oliver.
