It's 3D Tetris. Interestingly, it was published by Electronic Arts (EA) before they turned into the greedy corporation we all know and love today.

It's simple, It's fun, and I can't say anything else that can possibly add to this.

There is a new version called BlockOut2 from 2007 in open source.

There is also one online for browsers that work ok.
