Actually, the episode itself picks up right where it left off, but didn't appear for two years. We can see that the characters have aged a bit. Martin has given over his practice to an incompetent woman and her overbearing husband. She doesn't seem to know anything but to prescribe medications in abundance. Fortunately, Martin can't leave yet and has been given permission to stay for a while. The couple (still weathering Martin's obsessiveness) is embraced by the populace. But there is sad news. Joan has died. So Martin is left without his usual anchor. He has all kinds of opinions about raising the child and drives Louisa crazy.

Dr. Dibbs has arrived in town and she is ready to take over Doc Martin's practice. Unexpectedly, they get rid of Martin's old receptionist and that's only the tip of the iceberg--she's also a complete incompetent who must have gotten her degree from a diploma mill or veterinary college! At first, the concerns are only minor but after a while the new Doc nearly dies due to her own stupidity and general incompetence. In addition to all this, a few other problems develop--Louisa is exhausted after giving birth, the PC might have testicular cancer and Aunt Joan dies! So, it's necessary for Doc Martin to stick around...at least for a while.
I never have understood the BBC and how shows like this are made on only a semi-regular basis. In the case of this episode, while it picks up just after the season finale, it actually came out almost two years after! Still, despite this, the continuity was there and it's a most enjoyable episode.

....Bert Large and PC Penhale are annoying and obnoxious. The show is apparently built around which one can be the worst. Louisa is still horrid. Why do we keep seeing similarities between the idiots around Mr. Douglas on Green Acres and the idiots in Portwenn?
