Isabelle Nélisse can't speak English, which is why she's not given much dialogue and speaks more with body language.
Mama is played by a man, Javier Botet, who has Marfan syndrome, which gives him a slender body and long fingers.
Mama's movement is not entirely CGI. Javier Botet has well-above-average range of motion in his joints. The only CGI on Mama is her hair.
Mama's appearance was inspired by painting by Modigliani, owned by Andrés Muschietti.
According to Jessica Chastain, Annabel's octopus tattoo describes her character. When Annabel found herself caught in a situation she did not like, such as the octopus, she would take off her tentacles and grow new tentacles to escape.
Doug Jones portrayed the title character in del Toro's film Pan's Labyrinth (2006), and the Thin Infected Man creature in Quarantine (2008), the same character Javier Botet originally plays in [Rec] (2007) and its sequels, as the Medeiros Girl creature.
Screenplay is written by brother-sister tandem Andres and Barbara Muschetti, based on a short, titled the same and also created by them.

In this film, Nikolaj Coster-Waldau takes on the roles of twin brothers, playing both father and uncle to the two girls. In Game of Thrones, although the circumstances are completely different, he also plays both father and uncle to the young king.

The trivia item below may give away important plot points.

In the opening scenes where the father is driving on the cliff with the kids, the licence plate of their car spells "no 1 dad", a joke by the directors as the dad eventually crashes the car.
