This game is a military first-person-shooter where players will use a wide variety of weaponry to kill thousands of virtual enemies.
Whenever an enemy is shot, a mild/moderate amount of blood will be seen. Sometimes a puddle of it may form around the dead body. On rare occasions, the player may encounter injured enemies trying to crawl away, leaving a small trail of blood on the ground.
Enemies can also be killed by stabbing them with a knife. However, this attack is very quick and it shows very little blood.
You will probably see barely any blood in the entire game.
A man is briefly shot in the back. This is not bloody at all, but instead sad and surprising because he is a good soldier.
Some dying soldiers on the ground, but nothing too graphic. This is by far and will always be the least violent CoD game in the series.
In the mission "Mind the Gap", a little girl and her mother killed in a truck bomb explosion. It is not graphic at all, but it may be disturbing for some. This scene can be skipped over, however.

Even though the rating descriptor says "Strong Language", most of it's surprisingly mild for an M-rated game. The overall language differs between the English and Russian dialogue.
In English, there're four uses of "Shit", two uses of "Bastard", one use of "Ass", and some milder expletives such as "Hell", and "Bollocks".
In Russian, there're two uses of "Fuck" but most players won't understand since it's not translated.

One of the characters, Captain Price smokes a cigar at some points. At the end of the game, he is shown lighting one up and inhaling it from a first-person-perspective.
In the mission "Persona Non Grata", the player briefly injects Soap with adrenaline.
In multiplayer, your "Callsign" can be a marijuana leaf. This is not obvious however.
In stealth missions, you will see enemies smoking like the mission "Eye Of The Storm" and "Stronghold".

This game is not frightening, but some of the action scenes and cinematics can get rather intense.
Many civilian deaths might be disturbing for many people. Especially, In a level, we see a lot of bloody dead bodies of innocent civilians, and sometimes you must hide beside them, which is quite disturbing. There's also a few executions in this level, too.
As a First-Person-Shooter, this game glorifies military violence by taking 'said topic and portraying it as entertainment, and there are parts of the game that surround themes of vengeance and loss. However, it also explores themes of honour, self-sacrifice, and soldierly camaraderie.
