Sex & Nudity

  • Some of the generic enemies models appear to be fully nude, but the corpse is heavily mutilated and mutated, so no genitalia can be seen.

Violence & Gore

  • Much more violent and graphic than the first game.
  • VERY violent and disturbing. Constant alien and human dismemberbent with plenty of blood and gore through the use of saws and beam-like weapons.
  • When Isaac fails to kill an enemy he's slaughtered by necromorphs. Every death sequance of Isaac is extremely brutal and graphic (getting dismembered, eaten or crushed, stabbed with large blades or chopped to pieces).

Profanity

  • Words like Shit, Bitch, Asshole, Hell, Damn, Goddammit, Jesus Christ, are used pretty frequently, Unlike the last game Fuck, Fucker, Fucked, and Mother Fucker are used constantly throughout. Also when ever you press the stomp button Isaac starts swearing like crazy.

Frightening & Intense Scenes

  • If the player chooses to beat the game on hardcore mode then the whole gameplay gets very intense and frightening because Clarke is allowed to save only three times, checkpoints are cancelled and enemies are very challenging.
  • Very frightening throughout. The necromorphs/aliens are very scary loooking and the game has a very tense atmosphere. The main character hallucinates about his dead girlfriend often, and she is very scary looking. There are suspenseful moments that may cause you to jump.
  • Probably one of the most frightening games that combines dark atmosphere with gruesome enemies, suspenseful music, atrocities and recurring twisted visions. Take extreme caution with this game as fear is its strong point.

Sex & Nudity

  • A large three legged spider-like enemy ambushes the player in a church. The enemy has a human female's face and two sagging breasts hang in what used to be the chest area. They're just for details, and have nothing to do with the gameplay directly.

Violence & Gore

  • In one infamous scene, Clarke gets inside a machine to make himself an eye surgery, with his eyes wide open and a large needle slowly descending and subsequently inserting into his right eye to insert a marker in his brain. If the player messes up the proceedure, the piston with the needle will drop onto Clarke's eye, drilling all the way into his eye and brain as he screams in pain before dying and plenty of blood is released. This scene is VERY intense and graphic.
  • Some examples of violence are: at the beginning, Franco is brutally transformed into necromorph. One disturbing scene in the beginning when Isaac's doctor committs suicide by slicing his throat. Extremely bloody. A tied patient is killed by a necromorph very brutally as his leg and head are severed. Some clinic doctors suffer acid damage to the face and die. A woman embraces a baby necromorph and both of them explode. Blood covers the entire room. Stross attacks Ellie and with a screwdriver removes her eyeball. That happens offscreen. Eventually Clarke kills Stross by stabbing his head with the screwdriver.

Frightening & Intense Scenes

  • Nolan Stross is an incredibly tragic character who is driven insane by the marker. He is constantly crying, murmuring to himself saying things over and over again and later attacks Ellie, destroying one of her eyes.
