The M.O. matches the work of a serial killer known as the Surgeon, who once nearly murdered Jane. Now that the Surgeon is behind bars, it seems one of his protégés is picking up where he left off.

Jane and Maura investigate the murder of a local college student, Danielle Davis, who was gunned down on campus in broad daylight. According to Danielle's father, she was the perfect daughter. She was attending college on a soccer scholarship and was well-liked by everyone. The investigation reveals however that Danielle had recently lost her scholarship due to an injury but somehow still managed to pay her $57,000 tuition. She had recently broken up with her boyfriend and may have been having an affair wit one of her teachers. When it appears that the may have also been working as a call girl, Jane believes the solution to the crime rests with finding her pimp. Meanwhile, Jane also has to deal with her mother whose attempt at greater independence has left her with a clunker of a car.

Something of a wedge is driven between Jane and Maura when they investigate the death of Adam Fairfield, the head of one of the oldest and richest families in Boston. Maura knows the family well and was once engaged to Garrett Fairfield, Adam's younger brother. Jane wants to treat this like any other case but Maura thinks she should cut the family some slack, particularly since she hasn't yet determined a cause of death. Jane thinks Maura wants her to go easy because they're rich and important. When Maura does in fact determine that Adam was murdered, she also finds a clue that leads to the killer - an cashmere sweater. Jane meanwhile learns that her mother Angela has taken to selling a tonic of sorts in local pubs.

Jane and Maura investigate the murder of a young woman who was found beaten, raped and strangled outside a gay bar. At the scene they find a flyer from an anti-gay organization and their first thought is that they are dealing with a hate crime. The victim was married and her same-sex spouse says everything was fine between them, though the fact that she was out at a bar alone suggests she may have been cheating. The victim had a resemblance to Jane who goes undercover at the bar, with Maura working there as a waitress to collect fingerprint samples from anyone Jane might meet. Meanwhile, Jane goes with someone from her yoga class - Maura encourages her he doesn't quite meet expectations.

Jane and Maura's plan to run in the Boston marathon is interrupted when they reach the 3 mile mark and find another runner collapsed on the road. They quickly determine that the man was shot but Jane is concerned that it not become widely known as it might induce panic among the half million spectators along the course. When a second man is shot at the 12 mile point, they realize the killer is likely a runner. Working with Frost and Korsak who are back at the office, they learn that the two men went to high school together. They also learn that the they, along with a third student, had gotten away with a vicious rape. Someone is clearly out for revenge and the police have to move quickly when they learn the third man is also among the runners.

Jane once again has to deal with Charles Hoyt, The Surgeon, a serial killer who had taken her prisoner at one point but was arrested before he made her one of his victims. They've found a body in the park of a man who has been missing for nearly 2 years. His throat is slit and it seems that Hoyt, who is in solitary confinement, has once again made contact with someone on the outside who is doing his bidding. The dead man's wife is still missing however and Jane has been told to stay away from the case. Meanwhile, Jane's brother Frankie has a new girlfriend. Lola, a waitress that even Jane has taken a liking to. Maura makes an unexpected discovery however.

The police investigate the death of Colin Doyle who is found murdered sitting behind the wheel of his car. More interestingly they find dozens of passports and ID cards in the trunk of his car. Clearly Doyle was an identity thief and a good one who likely bilked a great many innocent people out of their savings and investments. Dr. Isles' autopsy reveals that the man was killed by by an ice pick. When it's determined that Doyle was actually the son of one of Boston's top-rated and most wanted Irish mob enforcers, Patrick Doyle, the police think it was likely an attempt to flush the elder Doyle out into the open. Maura's examination of the dead man also reveals something completely unexpected - that he is her biological brother. Meanwhile, Frankie Rizzoli arranges a plumbing job for his Dad at their favorite pub - and ends up working with him overnight.

After an undercover cop working a narcotics-smuggling sting is shot dead, the killers next turn their sites on Boston homicide headquarters, leaving Jane, Maura and Frankie Jr. trapped in a fight for survival on their home turf. It's a situation that will force Maura to challenge herself like never before. On the home front, Jane's parents are excited that their oldest child, Tommy, is being released after serving time in prison.

Three months have passed since Jane was shot and she hasn't yet recovered. There is a ceremony honoring her bravery and Maura convinces her to attend. The evening ends in tragedy when a U.S. Army Private, also being honored for an act of bravery while on a tour of duty in Afghanistan, is killed in a car bombing. Despite not being cleared for duty, Jane desperately wants to get in on the investigation. Meanwhile, Jane learns that her parents are getting a divorce. Jane renews her acquaintance with an old high school friend.

Jane and Maura's mud bath at the spa is interrupted when a man brings in a pregnant woman who has been stabbed. She dies but Maura manages to save baby. The woman's husband, a commercial fisherman, was on his boat when she was killed but says he wasn't aware that his wife was pregnant. The case gets more that just a bit complicated when it turns out she was a surrogate, carrying a child for a couple who were unable to have a child on their own. It leads them to a fertility clinic run by a Dr. Hanson who is less than cooperative. Jane's mother meanwhile is driving her crazy trying to redecorate Jane's apartment and then holding a yard sale there. Maura has an idea.

It's Fleet Week in Boston and Jane and Maura have a vicious serial rapist on their hands. All of the evidence points to a sailor as the culprit but with thousands of sailors in town, they have their work cut out for them. Detective Frost also has to deal with his father, Admiral Frost. Their first suspect is in the brig when the second attack occurs but the second victim was attacked in her hotel room and closed circuit video of the lobby helps them solve the crime. Maura meanwhile takes an interest in Angela Rizzoli's hunky mechanic, Giovanni Gilberti. Maura is looking for a physical relationship but Giovanni decides that she is his soul mate.

Jane Rizzoli and Maura Isles find themselves investigating a death that occurred during the re-enactment of of a Revolutionary War battle organized by a university professor, Dwayne Cravitz. They soon determine that far from being an accident, the shooting was from a sniper who was actually out to kill Cravitz. A DNA sample leads them to believe that the shooter is someone from his past. Maura meanwhile is thrown for a loop when her mother, the rich and beautiful Constance Isles, drops in for an unexpected visit. They have a strained relationship however with Mrs. Isles not quite sure how to be a mother to her daughter and Maura feeling intimidated and reduced to babbling in her presence.

Jane and Maura investigate when a man is killed during what appears to be a robbery. Evan Dunbar is killed and son Chris is badly beaten leaving him with broken ribs and a gash on his face requiring 22 stitches. Their immediate suspect is Arthur Dunbar, Evan's brother, who was released from prison just 6 weeks ago and who was imprisoned largely on Evans' testimony. DNA testing on rats however lead them down another path, one that includes the importation of illegal drugs and a kidnapping. During all of this, Maura is more than a little distracted when the love of her life, Dr. Ian Faulkner, drops in to see her. He doing good works in Africa but thanks to Jane's nosy mom, Jane learns that Ian is wanted for questioning by INTERPOL.

Jane and Maura investigate the murder of Melissa Joy Black who is found dead, tied to a pier at the port with an ice pick through the heart. Maura is concerned that it may be a mob killing with links to her biological father Paddy Doyle and so calls in another medical examiner, the fastidious Dr. Pike, who finds a micro SD card in her stomach. Melissa's sister tells the police her father was injured on the job 6 months ago and will never recover. Melissa likely got a job at the docks after the union blamed their father for the accident claiming he was drinking on the job. A wounded Doyle shows up at Maura's house demanding medical treatment but denies having anything to do Melissa's death saying someone is trying to frame him. After Doyle's right-hand man is found dead, the police think a war over control of the docks may be underway. Maura meanwhile is spending time with Jane's brother Tommy. Vince is pushing Jane to take the mandatory sensitivity training and she goes out of her way to evade the training officer.

Jane and Maura investigate the death of a young man, Graham Randall, who was killed in a prison washroom where he had gone to change after posting his bail. He was stabbed to death but while examining the body in the infirmary sees serial killer Charles Hoyt who is there for cancer treatment. He tells her a tale about a murder that occurred in 2005. Jane finds a cold case - a family of four that vanished into thin air - that could be what Hoyt was talking about. Maura's autopsy of Randall reveals something quite peculiar - teeth in his stomach that belong to the victims from 2005. Meanwhile Maura is fretting over what to buy Jane for her birthday and Frankie writes his detectives exam.

After the murder of much-loved community leader Elmore Gregory, Dante Morgan steps forward to identify the shooter as a gang leader, Terence Jones, also known as Little T. On the night before the trial, Dante leaves his police protected hotel room and is found dead the next day in a gang-ridden neighborhood with "snitch" written on his forehead. With the main witness out of the way, the only other evidence against Jones was obtained using information from one of Vince Korsak's confidential informants. When Vince says he can't produce him for the court, the judge gives the prosecution 48 hours of the case will be dismissed. Meanwhile, TV's Bill O'Reilly is in Boston to cover Little T's trial. Jane's mother Angela is a big fan and at a book signing she gives him Jane's cellphone number.

Jane's younger brother Tommy seems to be settling down. He's getting work here and there and is moving into his own apartment. He still sees Maury from time to time but she would like to keep their relationship a platonic one. Meanwhile, the homicide squad investigates a bank robbery where the manager was shot by one of robbers for no apparent reason. They immediately cross swords with the FBI who also have jurisdiction and Det. Barry Frost in particular seems to have it in for the Special Agent in charge, Anna Farrell. This is the second robbery committed by these men and they soon find the getaway vehicle with one of the robbers inside, shot to death. It all becomes very personal for Jane when her brother Tommy's fingerprints are also found in the vehicle.

After her boyfriend Casey Jones is unable to get to Boston, Jane takes Maura to her dreaded high school reunion. She hates most of the people there but one former classmate that she liked, Steve Sanner, says he would like to discuss something with her. Before he can do so however, Steve is shot and killed while taking a walk on the football field. The killer used a steel wool-filled water bottle as a silencer. He worked for another high school classmate, Rory Graham, who says he wasn't having any trouble at the office or with co-workers. Based on his phone records, they suspect he may have been having an affair with another former classmate, Debbie Tibbet, but when she is found dead her husband tells the police she had recently become very secretive. A third dead classmate gives them the clue they need to identify the killer. Meanwhile Vince Korsak's ex-wife is back in town wanting money for a new enterprise. Vince would like to see his 16 year-old son Josh.

Jane and Maura investigate the stabbing death of Denise Ryan, who stumbled onto the stage at her daughter Dakota's dance competition with a pair of scissors in her chest. According to the dance studio owner, Coach JJ, Denise wasn't like the other moms and didn't get to know her very well even though Dakota has been going there for two years. The husband is eliminated as a suspect but the police responded to a disturbance at the dance studio two weeks ago when two of the other moms, Yvonne and Kim, got into an argument. When they learn that Denise had no credit history whatsoever, they realize that the plastic surgery she underwent was likely to hide her true identity. Korsak meanwhile has been avoiding his ex-wife Melody who wants him to co-sign a lease so she can open her yoga studio. He learns that his stepson Josh has been arrested for shooting a policeman with his own gun. Joss is angry and refuses to speak to anyone.

Jane and Maura investigate a warehouse fire in which a fireman, Craig Hill, was killed. Maura finds that the fire likely started from a heater that had been tampered with. The building was a denim factory and the owner, Arnold Whistler, had a number of sexual harassment lawsuits filed against him. Meanwhile, Maura' and her mother Constance are the victims of a hit and run driver. Maura escapes with a few scrapes and bruises but Constance is seriously injured. When she finds her biological father, mobster Paddy Doyle, visiting her Jane begins to wonder if there's a connection. When they locate the car however, they find direct evidence to the warehouse fire leading the police to think that someone killed the fireman and then set out to eliminate Maura who must have come across some critical information.

Jane and Maura still aren't on speaking terms but are forced to work together when a college student, 25 year-old Rachel Lawson, is found strangled in the university's underground tunnels. She had also been renting a car every weekend for the last 20 weeks and was aware that her parents has separated. She had also spent a good deal of money on yoga in recent months. The yoga master, Sensei Matta, has recently bought 20,000 acres in Western Massachusetts and has a secret - one that lead to the death of Rachel Lawson. Meanwhile, Jane is surprised when her father knocks on her door. He has big news: he's going to be remarried - to a 28 year-old - but wants a church wedding so needs to have his marriage to Angela annulled. Tommy, who introduced his Dad to the woman, is having his own particular problems with the nuptials.

Jane and Maura investigate the murder of an army veteran, 29 year-old Bernard Avery, who was killed in an alleyway with a hatchet. He was homeless, one of many ex-servicemen now living on the streets of Boston, and suffered from PTSD. When Rizzoli and Korsak visit a psychology center where Avery had sought help, Jane is surprised to her old friend, Casey Jones working there. He's been back for several months but refuses to tell her anything more. The police soon have a second dead homeless vet and Jane's informant, Rondo, thinks he's found a witness. Meanwhile, Maura meets Dennis Rochmond and after saving his life, he sends her orchids. She also discovers her wild side.

Jane and Maura investigate the murder of 25 year-old Victoria Green who was found posed her on a bus stop bench and made up to look like a toy doll. They first suspect Herbert Buckley who was recently released from prison and has a fetish for dolls but that proves to be a dead end. It's only the first of several murders and it seems that the killer is finding the women on the public transit system. Maura finds a sliver of ash wood in the first victim's scalp but the blood on it is 20 years old. It proves to be the vital clue that leads them to the killer. Meanwhile, Korsak find a well-trained service dog who turns out to belong to someone at the veterans center where Jane's friend Casey works. Maura returns the dog and learns Casey's secret. It's left to Jane's mother to get Jane and Casey together again.

Jane and Maura investigate the death of a young woman whose body is found in a back alley dumpster. The killer apparently burned her hands to stymie her identification but the police find a driver's license on her. Maura determines that she was asphyxiated and suffered a severe blow to the head. Meanwhile, Jane is after Maura to locate her biological mother and it turns out she already has. She is Dr. Hope Martin a world famous physician who has done a good deal of forensic analysis. Jane arranges for her to come in and try to identify the dead girl. She turns out to be everything Maura could ever hope for in a mother but is determined that she should never know about their connection. As for the dead girl, she is identified as 27 year-old Emma Spencer, a legal intern who worked at the DA's office. She was investigating the murder of a friend who disappeared when they were young teenagers and whose remains were only recently discovered. She had also taken a leave of absence to work as a nanny for Tom MacGregor who is running for Congress

It turns out there are so many mysteries surrounding this victim that Jane has a tough time figuring out which trail is the one to follow. Fortunately one offers the opportunity to tease Angela's boss. Lydia returns for her baby and Angela has a plan to work things out.

Jane and Maura investigate the death of a young woman who is found washed up on the beach. Maura confirms that she drowned and manages to trace the exact location by analyzing the water in her lungs: a tank in an old fish factory. Jane is somewhat upset to find that an old flame, police Lt. Rafael Martinez, is back in Boston. They had worked together in the narcotics division but their relationship ended after a confidential informant had been killed and Martinez left to join a special task force some 8 years ago. The dead girl is identified as Brenda Thompson, a journalism student who was pursuing a drug story on her college campus. When Jane finds Lt. Martinez's business card among her possessions, it seems that he was using her to get information. Meanwhile, Maura learns that she may be in danger owing to her father's activities.
