Character error 

When offering to take her grandson to a folk festival, John's mother mispronounces the name of the city, Kutztown, which is in eastern Pennsylvania and quite a long drive from Pittsburgh. Pennsylvania residents would be well aware of the correct pronunciation. The U in Kutztown has the same sound as the OO in "book."

Continuity 

Throughout the movie the license plate of John's car change varying in numbers and letters.
The Prius' tail light can be seen intact several times after the scene where it got damaged.

Laurie's inmate number on the outside of the lab envelope is "PA-24-775", but on the lab results after he opens the envelope, it's listed as "PA-24-9876"

Errors in geography 

The zip code 15909 is in Johnstown, PA, not Pittsburgh.

The envelope with the labwork shows the Allegheny County Jail's address as "3256 2nd Street". There is no such address in the city of Pittsburgh. The jail is actually located at 950 2nd Avenue.

When John and Lara bypass the zoo exit they continue on the Highland Park Bridge. Shortly after, after the car avoids a truck and spins out of control, they come to a stop. As they exit the vehicle, you can clearly see Chestnut Ridge mountain in the background putting them on toll Route 43 outside of Uniontown, PA. This is over an hour-and-a-half from the Pittsburgh zoo.

When John and Lara leave the hospital running from Pittsburgh police, they are suddenly in Downtown Pittsburgh. But in reality, they were in a hospital in Oakland, which is a few miles from Downtown.

Factual errors 

Allegheny County Jail does not allow inmates to have contact visits.
Allegheny County Jail is inexplicably, erroneously described as "the largest jail in the world". In reality it only houses approximately 2,000 inmates. By comparison, The Twin Towers Correctional Facility in downtown Los Angeles is the world's largest known jail facility. It has 1.2 million square feet and houses almost 4,500 inmates.
Lara is held in the Allegheny County jail for three years after her conviction. Convicted murderers are not held in county jails in PA. She would have only been there temporarily for a pending hearing in Allegheny County Court. Shortly after her conviction, she would have been transferred into the custody of the PA Department of Corrections and transported to the State Correctional Institution at Muncy on the other side of the state for classification to either SCI Muncy or SCI Cambridge Springs.

Continuity 

When John and Lara are sitting on the ground next to the car we can see part of Johns digital watch when their hands meet. The numbers 0.00 are visible and not changing. Later the timer is running again.

The elevator key John stole in his college and treated at home is white and has different teeth pattern than that (bronze) he used in the prison and which is shown him during interrogation.

Factual errors 

Venezuela, the country where John and Laura escape to, does actually have an extradition treaty with the United States.

However, Venezuela has been known to refuse U.S. extradition requests, despite such treaty, along with many other countries as Bolivia, Ecuador, Iceland, Nicaragua, Switzerland and Zimbabwe.
The concept of using air pressure to unlock a car door, let alone from a tennis ball, was debunked on MythBusters in 2007.
When the guard shows John the broken-off bump key that John had used, it isn't a bump key. On a bump key, all the "teeth" are the same height, the "teeth" on this one aren't.

When the escaping couple board their flight they pass through Canadian immigration. There are no such exit controls in Canada.
Besides that a photo of Dr. Azahari, a Malaysian bomber mastermind who was killed by the Indonesian police in November 2005, still appears in the computer screen of the immigration officer at the airport--third columns from the left, and the the Brennans' tickets date were some day in 2004, two detectives who observes map and photos on their office's walls noticed that one photograph was a picture of Haitian presidential palace. But, one officer said that the palace was already collapsed because of the earthquake. The only earthquake that has made the palace collapsed was in January 2010 and the Haitian government decided to demolished it in April 2010. Also, the Lara's passport seemed to be issued around 2006, two years ahead of her plane ticket.

Plot holes 

For security reasons, inmates in PA are never notified when they are going to be transferred or transported between facilities. To prevent the exact thing that John attempts - to help an inmate escape.
Modern diabetes management will tell a patient immediately if their glucose levels are out of the ordinary. Results are not monitored and delivered to a county jail by an outside lab. If a diabetic is sick enough to require immediate hospitalization, they would have obvious physical indications. The entire escape plan hinged upon these fallacies.
