Goofs

Audio/visual unsynchronised 

When the prisoners in the pit shout 'rise', their lips don't move synchronised to the shout.
When Blake tries to appeal to the officers guarding the bridge, he tells the kids to go back and wait on the bus. When he says "Just go back and wait," his lips are not moving.
When they take over the stock exchange, the bad guys enter the place shooting. It is noticeable that their machine guns are not shooting and yet we hear the noise and we see the ricochet of the bullets against the monitors in the ceiling.
When Bruce gets up to leave from his meeting with Fox in Fox's office, Bruce says 'No' before his lips move.
When on the plane the CIA Op, attempting to get information out of Bane's men, fires a handgun near the head of one of the men; however, the slide on the automatic does not move despite the sound of the gun firing, nor is a spent shell seen ejecting from the gun.

Character error 

When Bruce Wayne is at the ball with Selina Kyle, he corrects her pronunciation of Ibiza to "Eye-beeth-a". Although it's pronounced that way by many English speakers, strictly it's pronounced "Ee-beeth-a".
When Bruce Wayne checks his computer in the bat cave and looks at the files he has on Seline Kyle, there is an Incident Report by the Gotham Police Department. On the top, it says "Klye, Selina", instead of "Kyle, Selina".
When Commissioner Gordon is going into the sewer, he tells Foley to call the DPW, standing for Department of Public Works. After the explosion, while the cops are standing around, Foley yells "Where's that DWP guy" mixing up the initials of the abbreviation.

Continuity 

The stock market exchange scene takes place during daylight. They established less than 10 minutes were needed to complete the exchange fiasco. Yet, when it's completed, its already night time.
In the stock market scene, Bane's henchman that has the tablet that is downloading information is seen putting it in his bag. In the next shot he can be seen doing this again even though the tablet was already put in the bag.
When Gordon is taken by Bane's men and put on 'trial', it is shown that Bane takes Miranda hostage. However, a later scene shows Bruce meeting Miranda and Fox, neither are hostages. Later still, when he first appears before Gordon, Batman asks where Miranda is, despite seeing her as Bruce Wayne in a previous scene.
As Blake runs into the hospital, he is seen to have a long shotgun with a wooden stock and a silver barrel. However, when he inside, he is seen to have a short black shotgun.
The last 1:56 of the countdown of the bomb actually last 2:21
During the final street chase, the characters pass by the same Jos. A. Bank clothing store no less than three times.
In the opening plane sequence, the main CIA operative fires a gun right by one of the prisoners' head out the plane door. His gun does not recoil from the shot, but a gun shot is heard. Later, we see him recock the hammer, an action that would have been unnecessary if the gun had actually been fired.
When Selina fights Stryver in the bar after giving him the fingerprints, he appears to be knocked out but when the SWAT team bursts in, he is nowhere to be seen.
During the stadium scene, crowd appears full when filmed from the field or the stands, but entire empty sections are seen in the aerial shots.
In the Batcave Bruce is looking for his thief Selina Kyle. In the first mugshot she is little over 66 inches (about 5'7") according to the height chart. In her second mugshot she is shown to be almost 72 inches (almost 6 feet tall).
During Stryver's sentencing Bane can be seen listening as Scarecrow mentions him, but when the camera angle changes to Scarecrow's perspective Bane is no where to be seen.
During the chase following the stock market robbery, the television headline alternates between "RETURN OF THE BATMAN?" and "THE RETURN OF THE BATMAN?" (with the "the" appearing and disappearing between different shots of televisions.)
After Bane and his men attack the stock exchange, vehicle crash barriers are deployed on the streets. The one we see deploying has its open side towards the stock exchange. But when Bane and his men leave on their motorcycles, the open sides of the angled crash barriers are pointed away from the exchange allowing Bane to use them as ramps.
As Bruce Wayne is meeting with Fox at his desk, his hand alternates between the top and the staff of his cane.
When Bane's team blow the tail off the CIA plane there is a solid sheet of hardboard across the opening - from inside the plane it is completely open to the sky.
When Bruce shoots the arrow at the target and startles Selena, it clearly hits right on the number 7 on the target. A moment later from Bruce's perspective, the arrow is in the target a full 6 inches to the right of the number 7.
When Catwoman shoots the cars blocking the tunnel, she beforehand puts on her glasses. In the next shot, she doesn't have her glasses on, just the mask.
As Batman begins to approach Bane just before their final confrontation, the same extra (a middle-aged gray-beard in a brown vest) can be seen fighting behind Batman only to be seen fighting behind Bane a split second later.
The river alternates from being frozen to liquid and back to frozen during scene transitions.
During the scenes at Wayne Manor (which was filmed at Wollaton Hall in Nottingham, UK) we see that the building is made of limestone. Yet during several scenes in the interior of the building, red bricks can be seen outside the windows, showing that the interior scenes were obviously shot at a different location or in a studio.
Just after Bruce Wayne loses his wealth, Blake is visiting him at home. At this point he has short sideburns. In the next shots his sideburns are long again.
During the chase after the trade heist, Bane separates from his henchman and passes Batman on his left side; Batman turns to see him to his right side.
While Bruce Wayne receives ride from Police officer, smudge marks on the passenger window changes between cuts. One cut shows a full hand print.
Towards the end, when Bane and Batman are dueling, just after Batman punches open one of the pipes of his mask, Bane punches a pillar while trying to punch Batman which damages the pillar. In later scenes, though, as they continue to duel, the same pillar is undamaged.
When Batman and Bane are fighting on the top step of the Town Hall, Bane charges at Batman and punches the column with such force that part of the column crumbles off. In the next shot from behind Batman, the column is back to normal.
During Bruce's meeting with Fox in Fox's office before being introduced to the Bat, the book on the chair next to Bruce changes position between shots.

When Bane blows a hole into the reactor bunker multiple pieces of debris are bouncing off the henchman.

When the truck that carries the bomb is forced to crash the railing and fall down to the road below, we see no light pole. But in the next scene, we see a light pole knocked over by the truck as it falls.

When Captain Jones turns to his last man, the hallway in front of him is empty. After the last man is killed and the camera switches back over to Jones, the hallway has several bodies in it despite neither Jones nor his last man shooting between shots.

Throughout the movie, Blake is seen carrying a Glock 17 pistol. When Gordon and Blake are talking in the apartment, you can see Blake's handcuff and spare magazine holder. The magazine in the holder is not a Glock magazine.
When Bane's men steal the CIA jet, the fuselage is clearly much smaller when seen from outside than interior shots suggest.
When Bruce is speaking to Alfred while sitting at a computer in the cave after he returns from rescuing Selina Kyle, his hair is lying unkempt over his left eye. However, when the camera moves to show Alfred and returns almost immediately to Bruce, his hair is neatly slicked back over his eyes.
In the beginning of the movie, when both the prisoners are loaded in the plane, we can clearly see that one has a black shirt and the other has a green one. In the next shots when prisoners are held with their covered heads out of the plane, both have black shirts.
During the Football Field explosion sequence, the grass goes from a dull brownish green turf before and during the explosion, to a deep lustrous green turf as Bane walks onto what remains of the field.

Crew or equipment visible 

Following the scene where Bane's sniper goes into the sewer and Commissioner Gordon gives chase, there is an explosion. The blast comes up through the manhole and the cop (on the left) clearly has flame retardant jelly covering his face.
Camera reflected in glass doors outside stock exchange during the fight between cops and Bane's henchmen.
Chalk markings "S/T" could be seen in a shot of the police-thug battle in Wall Street. They were believed to be the markings for the police actors to know where to standby before film rolled.
When Bane is reading the letter written by Gordon, there is a light skin-tone colored microphone wire leading down the back of his neck. It continues to protrude repeatedly, especially when he turns his head to his left.
When Miranda leaves the bank, two men are reflected on the door on the right.
As the shot pans over the hidden "Bat" near the bottom of the screen can be seen pieces of lighting equipment.

Errors in geography 

During the Stock market battle, the camera pans back and shows a banner hanging with NYC on it.
When the Gotham police force takes on Bane's recruited prisoners, a banner clearly marking "NYC" is visible in the background.
When Robin is heading to the bat cave using the handwritten coordinates, they reveal that the bat cave is actually somewhere near Los Angeles, California.

At one point, Wall St Subway station is clearly visible in Gotham.
When Selena tries to leave Gotham, at the airport, an Easyjet plane can be seen. Easyjet is a British low-cost airline that only operates in Europe therefore the airport clearly isn't in the United States.

Factual errors 

In the football stadium when Bane grabs the microphone off the Official, it is the back judge who would not have a microphone - only the head referee would have a microphone. Further, it's not a regular microphone, it's a headset used for communications between coaches, and between the booth official(s) and field officials. Those systems aren't connected to the PA system, though it can be done if a need comes up. Obviously Bane wasn't coordinating with the PA control booth, he just picks up the headset and talks in to the mic part as if it were hooked in already.
Poly isobutylene is referred to as an explosive in the film but it is not explosive. Butyl rubber is used to make C4 more pliable and less prone to detonation.
When Bruce Wayne was checking his computer in the bat cave for the first time, one of the articles about Selina Kyle spelled the word "Heist" wrong. It said "Hiest" instead.

Incorrectly regarded as goofs 

In "Batman Begins" Lucius Fox says about the Batsuit: "This sucker will stop a knife." In "The Dark Knight Rises", Batman gets stabbed. However, in "The Dark Knight", Fox gives Batman a new suit that makes him more agile but also "more vulnerable to knives and gunfire". If Batman kept his old suit, it's likely that the knife would've been less effective.
Early on in the film Bane makes a big point about how he was born in the dark and never saw light till he was a grown man. Later when we see the pit it is not only well lit but we get another speech about how the view of the sky is a key part of the prison's design. He never said he was born in the prison. Simply in the dark.
When Batman and Selena are on the roof, Los Angeles landmarks are visible including the U.S. Bank Tower and One Wilshire. This is not an error, as many landmarks that are obviously from other cities (Manhattan island, Brooklyn Bridge, Heinz Field, Sears Tower, etc.) are present throughout the trilogy.
Throughout the movie, the Gotham City Police Department uniform and cars show GPD and GCPD inconsistently. It is not uncommon for police forces to change vehicle liveries and/or styles of their uniforms every once in a while. New vehicles would be added to their fleet and given the new livery - to change every other older vehicle in the fleet to the same new livery would not be cost-effective.
It is not possible for Fox to know in which hour or minute exactly the nuclear device is going to explode as he didn't know at what time it was armed and countdown began. However, he was there when the reactor was activated, so he'd have a fairly good guess.
Midway through the movie, there is an aerial shot of a Tumbler (Batmobile) being slowly driven down a snow-covered street. The vehicle features a set of 4 rear wheels, yet only a single set of tire tracks are left in its path. However given that the Tumbler is a real vehicle constructed for the movie (neither a special effect, nor fake constructed over another vehicle) this is probably how the vehicle leaves tracks at low speed. (Six-wheel drive is unlikely to be efficient in slow city driving.)
After James Gordon is injured after meeting Bane the first time, John Blake visits Bruce Wayne and tells him that James Gordon was babbling about an underground army and a masked man called Bane. Bane never reveals his name during James Gordon's visit in the sewers. However both the CIA man at the beginning and Alfred's briefing indicate that Bane is a fairly well-known terrorist and mercenary and as the Police Commissioner of a major US city it's quite likely that Gordon would have access to counter-terror reports and briefs and would recognize Bane from them.
After saving Selina from Bane's men on the rooftop Batman parks the Bat in the cave followed by the scene where Alfred tells him about the letter and then leaves. When Bruce Wayne returns from the prison and saves Fox with the help of Selina and Fox asks where he had parked the Bat, it is parked on the rooftop of a building rather than where he had left it, in the cave. Batman most likely flew it onto the rooftop on his way to meet Selina in the subway, even though we don't see him travel there.
At the start of the film, when Selina Kyle inspects Bruce Wayne's desk, a picture of Rachel can be seen next to a picture of Bruce's parents. Later on Miranda Tate inspects a similar set of photos after Bruce broke into his own house. The second set of photos were different as they were not in Bruce's bedroom but more likely a downstairs lounge area.

Revealing mistakes 

One of Bane's mercenaries (on the far left of the rooftop fight scene with Catwoman and Batman right after it shows Daggett) falls down without being hit.
In the final fight sequence, when Bane sees Batman and starts his speech, two people fighting behind him clearly aren't connecting with the punches.
The final scene of the movie shows Alfred sitting in a café in Florence reading the Italian newspaper "La Repubblica". The newspaper is printed in black and white. That's the way La Repubblica is printed in foreign countries. In Italy and therefore also in Florence it can only be bought printed in color. This would be very obvious in this take because on the left and on the right of the big "La Repubblica" top line is always placed colored advertisement.
During the shootout in the alley between the cops and terrorists after Selina meets Stryver in the bar, one of the SWAT team appears to already have a bullet hole in the back of his flak jacket exactly where the sniper shoots him moments later.
During the stadium scene in Gotham City, at one point a logo reading "UPMC" (University of Pittsburgh Medical Center) is clearly visible next to Bane's head.
Many instances of Bane's henchman attacking one at a time and not reacting at all when being ambushed.
When Batman saves Blake from the ambush by Christopher Judge's character, he fights the henchmen. The guy to the very left never even encounters him yet is flown backwards and falls down as if he was attacked by Batman.
On the City of Gotham police report as seen by Bruce Wayne in the bat cave, the subheading lists Motor Vehicle Report: Los Angeles
When the captured plane's fuselage is dropped it falls straight down towards the road below rather than moving forward as it should, given it's being towed by another plane.
When Selina is fighting the goons on the roof and Batman comes to help her, her shoes go from being high heels to flats (especially noticeable when she runs for the Bat) - despite clearly making a point of her shoes being heels in the prior scene with Daggett
The seasons throughout the film are portrayed inconsistently even with the passage of time. This is evidenced via leaf foliage shown inconsistently throughout the film. For example, during the "winter" scenes on the isolated Gotham, many streets have a dusting of "snow" only to have trees with green leaves visible a block away.
In the football stadium explosion scene, the camera quickly shifts from a ground view of the stadium collapsing to a helicopter view. The end-zones of the football field change colors when it cuts.
In the first scene featuring the Bat, shadows cast by the Bat's supporting cables can be seen moving on the building in the background. These are most visible in the shot of the policeman's hat being blown off. The wires themselves were digitally removed in post production.

The goof items below may give away important plot points.

Audio/visual unsynchronised 

When Bruce returns to Gotham and meets Selina to give her the Clean Slate, when Bruce is speaking and the camera angle changes a couple of times to behind his right shoulder, his lips are not moving.

Continuity 

Throughout the trilogy, Batman noticeably wears thick black make up around his eyes when he has his mask on. However, when he returns The Bat to the cave for the first time, he talks with Alfred without his mask on, yet there is no sign of any make-up.

Continuity 

When Talia says goodbye to Bane, she places her right hand on his mask. In the next shot, her left hand is on his mask. Then in the last shot, it is her right hand again.

Factual errors 

When the bad guys are taking over Gotham, there are a number of bridges blown up. Some are suspension bridges that eventually have a small section of the center of the span blown away. A simple understanding of engineering would prove that stability is absolutely dependent upon the tension of the suspending ropes that stretch over the tops of the supporting towers. Yet, these bridges still seem to dangle out far beyond the towers with nothing to hold them up. With the cables cut, everything beyond the tower should have fallen immediately.

Incorrectly regarded as goofs 

After Bane takes the cowl off of Batman you can see Bruce's hair and then we see Bane toss the cowl down. Moments later we see Bane's men dragging Batman but his cowl happens to be on his head and with no harm done. This is because only the front part of the cowl is ripped off, leaving the back intact and you don't see his face as he is dragged away because of shadows hiding it.

Incorrectly regarded as goofs 

At the end, when they found out its program was tampered, Fox was running analysis on "The Bat", which would have been obliterated inside a nuclear explosion. This Bat however, is the original model, light blue, shown to Bruce by Fox during their meeting. Fox explains that "it does come in black," indicating that it is quite possible he had more than one of these vehicles in his possession at Applied Sciences at that time. Considering this, any software patch used by Bruce to fix the autopilot issue would have transferred onto all of the models, making it quite possible that the vehicle seen at the end is not the one piloted by Batman during the film.

Incorrectly regarded as goofs 

3,000 police officers are trapped underground for 3 months, kept alive by food parcels, yet when they are released, not one of them shows signs of significant beard growth. However, it is possible that they received razors with their food.

Plot holes 

Daggett's plan for hijacking Wayne's company could not have happened. Not only would it be fairly evident that Wayne was not on the trading floor that day, the fact that a terrorist had seized control of the Stock Exchange and hacked into their network would have cancelled all trading that occurred that day and called any trades that went through into question.

Revealing mistakes 

In the final scene, when the bomb is shown ticking closer to zero, smoke is seen emanating from it slowly as if the bomb were stationery. But in fact it is hanging by a wire and carried away into the ocean by The Bat at high speed, so the smoke should travel much faster.
