- Dr. Jack Gallagher treats a delusional husband and wife.
- Open with the docs going on rounds. Jack doesn't realize he's supposed to come with.
Cut to Dr. Moore examining seven months pregnant Melissa. She fell and began having contractions.
On rounds the doctors notice there are no patients. Surprise! Jack has them outside doing three-legged races (seriously) and the doctors are going to take part.
Moore does an ultrasound and looks confused.
Jack and his patient partner win the three-legged race. Everyone has fun. Moore shows up and tells Veronica he needs a psych consul. Despite all the other characteristics of pregnancy hormone levels, belly, etc. there is, in fact, no baby inside Mellissa.
The decision is made not to tell Melissa the truth just yet.
Veronica's husband Denny shows up at the hospital. He is a struggling musician who has arrived to play for the sick kids. Veronica seems bothered by his sluggish career.
This is Melissa's first "pregnancy." She's been married to her husband, Richard, for two years and is in town visiting his parents. Richard is on his way. Jack gets a call from Becky and leaves the room. Melissa tells Veronica that Richard is also their OB/GYN. Jack talks to Becky, who does nothing but breath heavily before hanging up.
While Jack explains there are several possible explanations for Melissa's belly enlargement, she walks over and says her water has just broken.
Richard comes to Melissa's room and is all kinds of fired-up. He orders a bunch of tests and a transfer of his wife but nobody listens to him.
Turns out Richard is a legitimate physician. Carl wants to let them go, but Jack isn't so sure. Nora comes to Melissa's room and assures Richard the maternity ward is full. Richard complains that the ultrasound and blood works were done wrong. Richard pulls out an ultrasound he did from his briefcase. Melissa tells Richard she lost control of her bladder and is embarrassed. While Richard reassures her we see he might be a little nuts. They will keep Melissa overnight.
Chloe gets a call from her significant other and Arturo hits on her openly. It turns out that Chloe is gay.
Richard conducts the ultrasound and tells Melissa that that the empty space is a head. We see that he is convinced there is a baby on the screen. Jack tries to stop Richard from taking Melissa out of the ward and he cold-cocks our director in the face.
Jack tells Nora he won't press charges. The staff believes the couple is an example of folly a deux: Richard's delusions have convinced an otherwise healthy person that she is pregnant.
In the break room Moore and Veronica begin making out. It looks as if they have been having an affair and Veronica wants to break it off.
Arturo tells Jack that Richard quit his practice when Melissa became "pregnant." The ultrasound Richard showed them was two years old. It was from Melissa, but from a previous pregnancy.
Carl disagrees with Jack's course of treatment and the conversation gets a little edgy.
Melissa admits to having been pregnant when she and Richard were first married. Richard's first wife could not conceive. Veronica tells her there is no baby this time.
Melissa's previous pregnancy was molar, meaning there were twins but the abnormal growth consumed the healthy embryo. Jack believes this put Richard's illness in motion. Becky calls again. More breathing and a hang-up.
Melissa tells Jack she doesn't think Richard loves her, that all he wants is someone to give him a baby. Jack tells her she is wrong and he will prove it to her.
Jack tells Richard the baby is in distress and they need to perform a C-section. Jack invites him to scrub in.
Carl tells Nora he doesn't like the idea of pretending to deliver the imaginary baby. He says he is trying to protect her and references that she put "more than her reputation on the line" when she hired Jack and references finances.
During the "C-section," Jack tries to get Richard to make the first incision. He can't do it, eventually falling into the corner in tears. Melissa comes over and they hug. He says he would never hurt her.
Richard voluntarily commits himself. Nora complains about Jack's dangerous techniques.
Denny plays Veronica a song her wrote for her. They discuss having a baby of their own and she jumps into his lap.
Malcolm tells Jack that a woman left something for him at reception. Jack runs through the lobby but can't find her. Jack pulls a flower taken from the hospital grounds of the package and smiles.
