A man seems unaffected when presented with the fact that if they take his advice and shackle patients to the floor, the patients would most certainly drown if the area were to flood.

This is quite a disturbing film that deals with numerous issues such as insanity, suicide, the holocaust, murder, split personalities, etc. Most of the action takes place in an insane asylum, and there are many conversations that detail gruesome murders. The main character has several flashbacks pertaining to his time in the war. The twist at the end, when the main character's identity and back story are revealed, is very unsettling and will probably upset some viewers (as it has to do with young children being murdered by their mother).

Rated R for: Disturbing Violent Content throughout, Strong Language and Some Nudity.
