the hospital faces a series of surgical emergencies 1)Mary the ambulance driver who has had a black out and is having brain surgery 2) Nick the patient with the burst carotid artery( which lexie is staunching) 3) Nolsten the stomach and thigh surgery with 4 kids 4) the real NAZI who has internal stomach injuries and Amanda is dealing with that 5) Ray ( the ambulance driver ) who is upside down with damaged internals the Cranial surgery has halted because the soft-wear and computer have failed - Rosie( the scrub nurse)offers to have a try as she did 3 semesters of computer science at San Diego ( "A party University "retorts mcdreamy ) she is nearly there Mcdreamy -tells her to concentrate as she has forgotten the restart button she remembers and they are under way -the atmosphere cackles between Rosie and mcdreamy the carotid has burst the room is full of blood , lexie uses two hands and can not reach the buzzer or phone - Nick reaches for the buzzer he can not and triggers more bleeding , he tries for the phone(reaching it with difficulty ) and they dial 8 - Nick makes joke while they wait meanwhile in the nolsten surgery the Ava / Rebecca has attracted attention to herself as complications arise which Hahn and Dane deal with ( meanwhile the atmosphere cackles as he flirts and she acts disinterested) Karev knows he will be disciplined and they go on with a number of complexities .It emerges that Hahn's policy is NOT to keep the the family apprised of the difficulties as it makes it difficult to manage the actual patient and Izzy finds it difficult .

the (NAZI)Paramedic is under the knife miranda has 2 problems - an operation which becomes increasingly complex and dangerous - and a belligerent husband George shuttles ,diplomatically ,back and forth to keep the husband( who is increasingly unhappy) sweet and barely succeeds meanwhile Miranda uses every surgical skill to keep the Nazi alive ( having announced she does not wish to be called the Nazi ) Meredith struggles to keep ray stable and quiet- she takes in scanner and then operates a 18 gauge ( huge ) needle to remove blood from the chest cavity to allow ray to breathe -the fire brigade free him .

meanwhile Ray is not sure he will make it and he sends a message to Stan's wife Mary about stan eventually they get him out and Sara comes to see him ( is there some romantic tension here ?) Miranda's husband Flounces out pronouncing the marriage is over -the Nazi comes to and complains about his Swastika tattoo being offset by the surgery george puts him right - miranda saved his life - Miranda could have allowed him to die at least 4 times - he owed his life to black woman he retorts about his belief system George gives him short shrift . Miranda is emotional but tough -Yang tears into her " for using her race against the Nazi " Nick is shuttled to and from the operating room as they await an opening - and lexie leads attempts to stabilize Nick using various techniques - Nick has a premonition of dying but lexie will not allow such thoughts - but it is all to late and he fades away.

Karev and Ava meet in an on call room , he wants sex , she wants to talk - how can you want sex after such a difficult surgery - we save lives that's what we do . "What do you want" asks karev you have a child and husband . She has no answer. he tells her what he wants and that he has another sex buddy and that she has got him in trouble. she has answer.

lexie is distraught and Yang surprisingly takes her under her wing and brings her round to Meredith's saying "she has no one " they start they crazy pogoining and Izzy and George join in Using Torres for proximity talks-Dane and Hahn go out for drink ( with her)
