The pilots were not wearing shoulder harnesses, so upon impact, their heads went forward, striking the dash, injuring the pilot and killing the copilot. (There were also holes in the windshield, but these could not have been caused by impact with their heads, because the windshield is very thick and is able to withstand bird strikes.) In reality, pilot wear shoulder harnesses, and prior to crashing, lock the inertia reels so that there will be no slack, thus preventing their bodies from going forward upon impact. In this crash, the shoulder harnesses would have prevented these injuries.
