Spoilers

The trivia items below may give away important plot points.

Gwen asks Jack what could have tempted him to open the rift and he replies "the right kind of doctor," meaning the Doctor from Doctor Who (2005). The sound of the TARDIS materialising is then heard.

The events in this episode take place (in Jack's timeline) immediately before the events of the Doctor Who episode Utopia (3.11), which begins with The Doctor stopping in Cardiff to refuel the TARDIS with the rift energy. At the end of this episode Jack sees the severed hand glowing and beeping and hearings the sound of the TARDIS. In Utopia, as the TARDIS is taking off again we see Captain Jack running towards it, and then we see him clinging to the outside of the blue box. Jack then goes on to join the Doctor and Martha Jones as they battle the Master and the Toclophane during the two part Doctor Who season 3 finale (the year that never was). His adventures with The Doctor explain his absence (Owen says Jack has gone missing though he shows up shortly after) in the beginning of Torchwood season 2.
