at the end a character says you got slapped by the D

Characters attempt to cut through rope by using belt still attached so they look like they are dry humping (comical)

Dale bashes Reds face in with a toilet seat. He ends up ok.

Red is shot in the stomach twice, somewhat comical manner he moans and Blood forms around the bullet holes.

After Saul believes Dale is dead, he gets up and we find out that the thug shot his ear off. He graphically picks up his piece of ear and tries to attach it back to the wound with blood coming out of it. (used for comedic effect, but still gross)

After Red runs over a thug, he blows his foot off with a pump-action shotgun for fun

A Ninja is shot in the fleshy part under his chin, his corpse is later shown laying in a pool of blood.

Ted is shown graphically burnt

Carol gets crushed by a car

a suicide bomber blows himself up and attempts to kill two other men

A man is shot in the head, falls against a window, and blood and brain matter splashes on the window.

A man's head is smashed with a hot coffee pot and his face is disfigured for the rest of the film. He is killed when he is run over by a car, once he is dead, the drive shots him in the leg.
