McGee is forced to choose between his NCIS family and his little sister. An impossible choice on many levels. He makes some bad decisions along the way, but helps to solve the case in the end.

Nepotism is successful

Twisted sister is an entertaining "gimmick" episode of NCIS, that remains enjoyable 5 years later. Casting of producer's daughter and Sean Murray's step-sister as his younger sister Sarah works quite well. That's the jumping off point for the key gimmick: putting Murray as McGee on the spot: sis shows up on his doorstep all bloody, half-amnesiac and thinking she's murdered someone. As McGee naturally tries to protect her, his status as government agent is in jeopardy since his team is investigating the case where she is clearly the prime suspect. Entire cast is convincing in dealing with this extreme situation, including bosses Mark Harmon and Lauren Holly who must make critical decisions regarding McGee. Of course, the closure of episodic TV irons everything out by episode's end, but I was captivated by McGee (and sis) facing seemingly insurmountable problems. Troian Bellisario, who dad had been putting in his TV series as far back as 15 years earlier (when she was just 5) in "Quantum Leap", is quite effective as a still unseasoned actress challenged with a nearly unplayable role, and she does team up convincingly with Murray. All in all, a tricky segment pulled off nicely.

Warning: Spoilers
MGee's sister is in trouble, amd he jeopardizes his career to help her. Co-worker Tony is his usual insensitive, immature, self centered self, offering no help or support for McGee, belittling him and, when bringing the sister in for questioning, calls her a "hot chick".. Boss Jethro supports McGee,refuses to accept his resignation and resolves the issue, solving the crime and clearing McGee's sister.
