The first time Cherno pulls the trigger on the revolver, the cylinder rotates so that the chamber holding the bullet goes from the lower left position into the upper left position, or the next chamber to fire when the trigger is pulled again. The next time Cherno pulls the trigger, the chamber with the bullet is in the lower position and moves into the lower left position, and would require two trigger pulls to enter the firing position, ending up in the starting position for the first trigger pull.
