Character error 

Jim, impersonating a phone repairman, tries to go into the colonel's office. Grigor, from several feet away tells him that he can't go in there. The guard at the door doesn't react, other than looking at Jim. He just stands there with his arms folded.

Continuity 

After Toland finishes photographing the red book, the IM crew is alerted to his departure by the slamming of the door. When they and the doc enter the room to save the lady who was forcibly drugged, the door is wide open.

Errors in geography 

21 minutes into and on: The plot is apparently set in a fictional country in the Eastern Europe. In their head office, the background map on the right is the western part of Japan.

Revealing mistakes 

When Rollin is "impaled", there is a hole in the back of his shirt; when Jim helps him out of the harness that simulated the effect, we see that the spike is hinged at its base, meaning that it would have ripped a slit in his clothing, not made the small hole that was showing.
