Victoria is injured when a prisoner, a doctor being held for murder by a marshal, tries to escape. Big John is left with no choice but to have the prisoner operate on her. He has to go against the marshal who believes Victoria will die.

Storyline

U.S. Marshal Ted Garnett arrives at the ranch with prisoner Matthew Kendall. Kendall is wanted for eleven murders but Big John agrees to let them stay overnight at the ranch. When Victoria and Garnett take food to Kendall, he tries to escape and during the brawl Victoria is severely injured. They decide to send for the doctor but Manolito arrives saying the doctor left Tucson and it would be a 2-3 day ride to reach him. Garnett told Big John that Kendall is a doctor and killed eleven prisoners of war by operating on them while drunk. After Big John has Kendall examine Victoria, he believes that she has broken ribs but the main injury is a bleeding spleen which must be removed. With time running out Big John agrees to free Kendall if he operates. Kendall reluctantly agrees. They have ether but they make many of the instruments he will need and Joe and Pedro scrub the area with boiling water. Big John handles the ether while Kendall operates.
