Character error 

In the hospital, the girl's flashback memories are from the point of view of whatever was chasing her. Her memory would have been what she saw, not what saw her, as it was later when she remembered her mother.

When Anna got into the car and realized it was not her mother, she could have easily opened the door and escaped.

Bruce Nelson upon arrival at the first crash uses an incorrect 11-82 code (property damage) for the crash, and then fails to use the 11-41 code for fire and ambulance.

Elvira calls Central Station to tell Ponch that she has some news. Instead of just speaking to Ponch on the phone, she has him visit her at the comic-book store, yet what she tells him could have easily been relayed over the phone. It makes sense that Elvira might want to see Ponch and Bruce again, but Ponch shouldn't waste time and gas without even inquiring over the phone what Elvira knows.

Continuity 

In the first crash, the white pickup with tar trailer is seen crashed when the yellow car is launched over the brown ramp car, then in the following scene we see it crash and the brown ramp car stop behind the trailer. Next, in the blue Oldsmobile with the white top, the woman driver is seen with both hands on the wheel, yet in the following scene the driver has their left arm out the window holding onto the roof. Finally, the location of the ramp vehicle changes from being uphill to being downhill.

The yellow Datsun Z-car with a black rear spoiler is seen behind Poncherello and Nelson when they respond to the first crash, and it is then seen stopped in traffic at the crash as they arrive. Additionally, the location of the crash has moved much farther away from the intersection than was shown when the vehicles were first seen crashing.

Errors in geography 

Poncherello tells Bruce Nelson they are assigned to Malibu for the rest of the week. The Malibu area is not within the CHP Southern Region which includes the Central station. It is in the Coastal Region so they would not be assigned to work there.

Factual errors 

When Poncherello is fighting in the water, both Linahan and Bruce Nelson do not assist him and instead just watch from the beach. As trained and sworn officers, they would have instinctively helped him.

The first crash takes place close to a t-intersection near the end. The yellow car that rear-ends the ramp car could not have built up enough speed to do so after it turned onto the road.

Revealing mistakes 

When the casket is opened, the body inside is tanned flesh tone and the head is lifted at an angle, yet seconds later when the face cracks it is gray in color and laying flat in the casket.

The same blue 4 door Chevrolet sedan is used for both traffic incidents when the hearse is fleeing. It was also the first car to spin out in the first crash when Anna ran into the road.

In the first crash, the brown car is a ramp car that has a ramp installed on the rear section to launch cars that rear-end it. It can be seen many times throughout the series. Additionally, the cars in the crash are all unoccupied when the yellow car is launched and crashes, yet in the following scenes the occupants are seen getting out of them.

As Poncherello and Bruce Nelson are leaving the report room after learning about the key, the wall to their left moves, as evidenced by the lights reflected in it changing position. This indicates the wall was a lightweight set piece, not real construction.

Spoilers

The goof items below may give away important plot points.

Plot holes 

Anna's mother is imprisoned in the secret room behind the bookcase, yet she can look through the eyes of the painting and see what's going on in the living room. CHP officers visit the house multiple times, and the mother sees them, yet she does nothing to try to alert them to her presence.

It looks as if the painting has hole in it, so she could have easily yelled, but even if there aren't holes, she could have banged on the wall to attract the officers' attention.

Revealing mistakes 

Woods is seen physically putting Anna into the room with her mother when the doorbell rings. He then puts on a jacket and answers the door. It is later shown that this version of Woods is a hologram. This also fails in that the hologram interacted with Poncherello when the real Woods was inside his vehicle making a getaway. It would be impossible for a hologram to physically interact with real people.
