Goofs

Character error 

After Home Mart cashier Jenny gets robbed of her cash register money and the thief also takes her ring, McCall grabs a new sledge hammer from the store to exact revenge and get the ring back. All the hammers are hanging between wall pegs with the UPC stickers on the handles facing out.

When the store opens the following morning and Jenny finds her ring in her register, McCall returns the sledge hammer to its place on the pegboard, wiping it off as he's doing so. Only he places the hammer with the sticker facing in, not out like all the rest.

Someone with OCD would never do that.

Continuity 

At the beginning of the movie, while getting ready for work, Bob dresses in slacks and a button-up shirt. He is wearing the same outfit while at work, but on his way home, when he gets off the bus, he is wearing blue jeans. Once he arrives at his apartment, he is wearing the slacks and button-up again.
While McCall and Alina are speaking in the Bridge Diner restaurant when they first meet, Alina removes her earphone on the left side. In the next shot, Alina's earphone and the earring have been removed on her right side not on the left side.
During the final fight, Ralphie turns on all the lights in the store and yet when McCall faces Teddy few lights are on.
Robert and Teri walk down the street in East Boston and then stop and talk. A woman and child walk by them twice in the same direction.
At the beginning of the movie, when Robert leaves his house he puts the napkin in his jacket's inner pocket but later when he takes the napkin out oh his shirt's pocket.
After Robert beats up the two detectives he takes the magazine out of the gun and then he cycles the round out of the gun and when the cartridge hits the ground you can tell it was empty by the sound.

Errors in geography 

The power sockets next to the mirror in the Moscow bathroom are UK style, not the Russian Type.
The whole "Moscow Mansion" is not in Moscow. First, there are no such mansions in Moscow. Second, the identification numbers on the security cars outside are clearly of the European standard with the blue field at the left side. Third, the windows are opening like in UK, or in USA - upside down.
At the end of the film, McCall is watching the sun set, difficult, if not impossible, from Boston. The sun is not rising because that would mean that the next scene of McCall on the beach takes place when the sun is down or coming up, which would be chronologically out of order.

Factual errors 

The quote shown at the start of the movie, "The two most important days of your life are the day you're born and the day you find out why." is attributed to Mark Twain whereas the source of this quote is actually unknown.
With the electricity out, McCall sets a booby trap consisting of propane tanks in a microwave oven. When the power comes on, the microwave starts and - precisely 40 seconds later, as McCall planned - the propane explodes. Several problems: With power out, the digital microwave cannot be set. Regardless of being set, a microwave doesn't "come on" when power is applied. And while McCall is very good at what he does, how he would know exactly when that particular propane-microwave combination would explode (right when the bad guy entered the break room) is impossible. (Great flick though!)
Exposure to a few minutes of car exhaust does not produce a coughing fit reaction as depicted in the movie. It would instead cause gasping, nausea and dizziness.
The "nail gun" McCall uses to kill Teddy in Home Mart is actually a Ramset Powder Fastening Systems gun. It's used to fasten wood- or steel-framing studs to concrete or steel. It fires 1-1/2 to 3-1/2 inch nails but for safety reasons can only be discharged if the bottom plate is pressed firmly against an object.
During the final "battle" in the DIY warehouse depot, one of the enemy activates night vision equipment. It does NOT make a high pitched sound when turning on - can only presume people have been playing too much Splinter Cell.
In the diner scene with the Russian in the power/construction outfit, sticking a dinner knife in the outlet would not have made contact much less short the main breaker even if it did make contact with only one terminal side.
When Robert has handcuffed the dirty cop inside the Jaguar, the cop should be able to stop the window from rolling back-up. He could possibly use his free arm to reach and pinch off or plug the garden hose behind him.
At one point, Robert is riding a Red Line train, but the interior is not correct. Red Line trains in Boston have much wider cars, and Robert is shown in a train that more like the narrower Orange Line train he was on earlier in the film (probably a cut of the same footage).
When McCall cuffs Frank in his car and forces him to speak by gassing him, he lowers the window with the remote. Cars that offer this function usually lowers all windows. (Cars with electric windows also have window locks to prevent their operation.)
McCall deals with one of the bad guys in Home Mart by piercing him through the neck with the saw on a pole pruner. Such saws though are quite thin and flimsy. While he could have dispatched the guy by sawing across his neck, no way does that saw blade transfix a human neck.

Miscellaneous 

(at around 1h 20 mins) A bad guy is chain smoking in an apartment. An overflowing ashtray is surrounded by empty cigarette packs. All the butts are Camels; all the packs are Marlboro. He pulls a Camel from a Marlboro pack and lights it.
When Bob is closing his wound using boiled honey you see an empty bottle of honey by the stove. Then you see a full bottle of room temperature honey in the bathroom with him.

McCall defeats the fake power worker in the diner, but takes a glancing shot across his inner right thigh. When he flees, he turns left down an alley. Teddy follows and tracks him by noticing a chest-high blood stain on the left wall immediately upon entering the alley. How did a blood stain from a wound on an inner right thigh show up chest high on a wall to the left? No way McCall put it there because he's clearly trying to avoid them and does for the rest of the scene. (It is possible he put his hand on it, and transferred the blood to the wall.)

Incorrectly regarded as goofs 

In his flashback, Bob wears his wedding right on his right hand instead of his left. (A wedding ring on the right hand signifies a widower.)

Plot holes 

When McCall cuffs Frank in his car and forces him to speak by gassing him, Frank should have no problems opening the door or lowering the window from inside the car, having one hand free.

Revealing mistakes 

When Masters checks his revolver after receiving a call from Teddy, all the primers (center part on the back of a round where the firing pin must strike to set it off) have all been dented. This means the bullets have been fired, and nothing would happen if the primers were struck again.
(at around 31 mins) A brief transition scene shows a Boston "T" Red Line subway car traveling from left to right. Frame-by-frame review of this scene reveals that this transition has been flopped because all the writing on the subway car is reversed.
