Selma Blair did almost all of her own stunts. For her underwater scenes, she spent 12 hours in a water tank, with only short surface breaks, for two days straight.
In between takes in Vancouver, press were granted access to the set. During Selma Blair's interview, director Rupert Wainwright made a joke she didn't like. In response, Blair reached into her shirt, pulled out a rubber "falsie," and flung it across the conference room at the director, deadpanning, "That's the Adrienne Barbeau part of the role."

Tom Welling's character, Nick Castle, is named after Nick Castle, the actor who played The Shape in John Carpenter's Halloween (1978).

Before Tom Welling was cast, actors considered for Nick Castle included David Boreanaz, Jesse Metcalfe, Matthew Davis, Henry Cavill, Adam Garcia, Michael Cassidy, Oliver Hudson, and Peter Facinelli. 'Matthew Fox' and Ben McKenzie, were also considered for the role and met with the director, but due to conflicting television schedules they did not read for producers.

Tom Welling had three weeks left on Smallville (2001) season 4 when he began shooting this film. Selma Blair often joked that the director kept two cameras running during his scenes; one for 'The Fog', and one for 'Smallville'.

During early development, Julia Stiles expressed interest in the role of Elizabeth. The screenplay was subsequently tailored, the character matured, and the role expanded. When Stiles did not come aboard, Maggie Grace was immediately cast, and the character of Elizabeth was rewritten to be younger and more innocent than in previous drafts.

When the remake was announced, rumors circulated the Internet that Charisma Carpenter had been cast as Stevie Wayne and Jessica Biel as Elizabeth. Later reports stated Carpenter was deemed too old for Stevie Wayne and Biel simply did not want to do any more horror films. Similarly, reliable websites announced that Chris Isaak had signed on to play Nick Castle, which was completely untrue as the character was rewritten to be in his mid-twenties.

Selma Blair was the studio's first choice for Stevie Wayne, but she wasn't offered the part because Julia Stiles was still attached to play Elizabeth. Blair and Stiles had acted together twice before (in Down to You (2000), and A Guy Thing (2003)). Blair was contacted when the role of Elizabeth went to Maggie Grace. Her contract closed with 48 hours, and she started shooting less than two weeks later.

In this version Elizabeth is the daughter of the town official. In the earlier version these characters were played by real life Mother and Daughter Janet Liegh and Jaimi Liegh Curtis. However in that version their characters are not connected, only Shere one scene and never speak on screen.
